As the organization continues to grow—including the development of the region’s first hospice residence—Evergreen is strengthening its internal operations to support sustainability, quality care, and long-term impact across the communities it serves.

The Opportunity

This is a senior leadership role responsible for both the delivery of care and the coordination of operations that support that care.

Evergreen Hospice is a growing organization where programs, people, and processes have evolved organically. This role will bring structure, clarity, and alignment across both client services and internal operations, ensuring that care is consistent, coordinated, and sustainable.

Working closely with the CEO, the Director of Care will lead across counselling, intake, and program delivery—while also strengthening the systems and processes that support day-to-day operations.

Core Areas of Responsibility

  1. Care & Program Leadership
  • Lead and support counselling, intake, and community-based programs
  • Ensure consistent, high-quality care across all services
  • Strengthen coordination between individual, group, and volunteer-supported care
  • Support development of a more structured and integrated care model

2. Client Care Coordination & Service Delivery

  • Oversee intake, triage, and client flow
  • Ensure appropriate service matching and coordination
  • Improve visibility into caseloads and service delivery
  • Support development of a clear client care coordination approach

3. Team Leadership & Clinical Alignment

  • Provide leadership and support to counselling staff and program team members
  • Support supervision structures and role clarity
  • Foster alignment, accountability, and collaboration across the care team

4. Operational Coordination & Systems

  • Strengthen coordination across scheduling, staffing, and program delivery
  • Improve visibility across calendars, caseloads, and services
  • Oversee use of systems (e.g., AlayaCare, SharePoint) to support care delivery
  • Introduce simple, practical processes to improve day-to-day operations

5. Quality, Documentation & Accreditation

  • Ensure alignment between care delivery, documentation, and policy
  • Support integration of accreditation standards into daily work
  • Improve consistency in documentation practices
  • Introduce light tracking to support quality and compliance

6. Volunteers & Student Integration

  • Work closely with the Manager of Volunteers to integrate volunteers and students into care delivery
  • Ensure clear roles, expectations, and supervision structures
  • Support a positive and structured learning environment

7. Organizational Growth & Readiness

  • Support alignment of current programs with future hospice residence needs
  • Contribute to development of care models and service delivery structures
  • Ensure systems and processes can scale with growth

What Success Looks Like (First 6–12 Months)

  • Clear coordination across counselling, intake, and programs
  • Improved visibility into caseloads and service delivery
  • More consistent client experience across services
  • Stronger alignment between care delivery and documentation
  • Improved day-to-day operational flow (scheduling, communication, systems)
  • Better integration of volunteers and students into care

Ideal Candidate Profile

  • 5–8+ years in hospice, palliative care, counselling, or community health
  • Experience leading or coordinating care teams
  • Strong understanding of client-centered care models
  • Comfortable working across both care delivery and operations
  • Able to bring structure without overcomplicating systems
  • Calm, practical, and solutions-oriented

Preferred:

  • Clinical designation (MSW, RSW, RN, RP)
  • Experience in clinical care settings
  • Experience working with students or multidisciplinary teams
