Family Caregivers of BC - Community Navigator

This is not a home support worker/care aide position.

Location: Prince George, BC | Hybrid: Combination of remote and community-based work

Travel: Regular travel within Prince George and surrounding communities is required. Access to a reliable vehicle and valid Class 5 Driver's License required.

Hours: Full-Time (35 hours/week) | Monday-Friday, 8:30am-4:00pm | Occasional evenings and weekends required

Position Type: 3-Year Term

Compensation: $34-$36 per hour

ABOUT THE ROLE

The Community Navigator provides individualized caregiver support, navigation, education, and connections to community resources through the Caregiver Rx Social Prescribing Project.

KEY RESPONSIBILITIES

• Provide caregiver intake, support, assessment, and navigation.
• Connect caregivers with community resources and services.
• Develop and maintain referral pathways with community partners.
• Maintain accurate client records in Salesforce.
• Deliver outreach, presentations, and community engagement activities.
• Support program evaluation and continuous quality improvement.

QUALIFICATIONS AND EXPERIENCE

• Degree in a related field or equivalent combination of education and experience.
• Minimum 3 years of experience in case management, navigation, or caregiver support.
• Experience working with rural, remote, and culturally diverse communities.
• Strong communication, partnership-building, and problem-solving skills.
• Proficiency with Microsoft Office, Zoom, and digital platforms.
