
Client Services Coordinator

Position: Client Services Coordinator – 12 month Contract Position
Qualifications: Post-secondary education in gerontology, health, social services or related discipline, working knowledge of the long-term care system including facility and community resources an asset; supervisory experience preferred. Nursing background is an asset. Must have a valid driver’s license and own vehicle.
Responsibilities:
- Engage with seniors on their service and housing needs
- Perform Community Health Assessments to determine program eligibility
- Develop service plans for clients
- Communicate with external providers
- Ensure all proper documentation is in place
- Work with Manager and Director to review resource allocation
- Attend meetings and training
- Other duties as assigned
Must have a Vulnerable Sector Police Check – not older than 1 year and a valid driver’s license, auto insurance and own vehicle.
