Full- Time Permanent 1.0 FTE

$67,776.80-$82,373.20 per year – based on 35 hours/week

Job Summary: The CDPM Nurse specializes in Chronic Disease Prevention and Management, providing expertise in self- management for individuals living with chronic conditions such as diabetes, COPD, hypertension, CAD etc. This role supports clients and families in healthy living, illness prevention and health education/promotion as part of BQWCHC comprehensive primary care services, delivered through both individual and group formats. The CDPM Nurse also contributes to the development, implementation, monitoring and evaluation of related programs and services.

Key Areas of Responsibility:

  • Perform assessments, triage, health education/counseling, history taking, and other clinical activities in accordance with the College of Nurses of Ontario standards of nursing practice and evidence-informed best practice guidelines
  • Provide high-quality client care independently through the use of medical directives and collaboratively as part of an interdisciplinary team
  • Collaborate with Nurse Practitioner and Physician group in the development, revision and implementation of evidence-informed medical directives
  • Deliver care in a trauma-informed manner that respects client self-determination and reflects an understanding of the social determinants of health
  • Provide back-up coverage in the general clinic nursing role, as required
  • Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services with a focus on individuals living with chronic conditions, caring for someone with a chronic condition, or at risk of developing one
  • Ensure program design and delivery is aligned with BQWCHC priorities and values including client engagement, accessibility for target populations and responsiveness to the social determinants of health

Qualifications:

  • Registration in good standing with the CNO as a Registered Nurse (RN) or Registered Practical Nurse (RPN)
  • RN must have a minimum of three years of experience practicing in chronic disease management; current certification as a Certified Diabetes Educator (CDE) is considered an asset
  • RPN must hold current certification as a CDE
  • Nursing experience in a community setting or combination of community, hospital or public health settings; experience in a primary care setting is considered an asset.
  • Experience working in team-based care environment.
  • Clinical leadership experience in chronic disease management is considered an asset.
  • Experience in program planning, implementation, delivery and evaluation is considered an asset.
