Job Summary:
As a Psychogeriatric Case Manager you will be part of a Behaviour Support Team that provides direct care, education and support to seniors who are demonstrating responsive behaviours that may be associated with various types of dementia, delirium, mental health, addictions or other neurological conditions and who are ALC or at risk of becoming ALC in the acute care and/or transitional care settings of the Ontario Health Central area. You will assist in the creation and coordination of a behaviour support network, you will provide direct service to support the person served, family members and/or care-partners to mitigate escalating responsive behaviours. A key role of this position is to coordinate and support the transition of the person served as they move to their next care destination.
What You Will Do:
- Assists persons served in the development and implementation of a Behavioural Support Plan tailored to the needs of the person served and issues designed to improve quality of life and assist in the mitigation of responsive behaviours. Identify and refer to resources specific to those needs and issues;
- Act as a liaison and client advocate among those involved with the client’s care including family, other caregivers, medical and other multi-disciplinary providers.
- Complete the Behavioural Support Assessment and provide timely and accurate record keeping and reports as required.
- Provide support, on-going training and consultation to colleagues, family members and/or other care-partners to assist in mitigating escalating responsive behaviours;
- Provide supportive counselling or referral and linkages to health care, programs, or service providers.
- Provide crisis management/intervention and assist in the development of crisis plans for clients.
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
- Maintain strict confidentiality and follow all other required policies and procedures.
- Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization.
- Attend and actively participate in all required training.
- Liaise, collaborate and coordinate with other professionals, service providers and partners
- It is an expectation of the position that most of the work will be conducted in hospitals and compliance with their occupational health policies and protocols will be required (associated costs will be reimbursed)
- Complete other duties, from time to time, as assigned by the Coordinator and/or Program Director.
What You Bring:
- Completion of a diploma/degree in Social Services/Human Services or equivalent;
- Training/knowledge of P.I.E.C.E.S., U-First approaches or willingness to receive training;
- Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions;
- Group counselling related to mental health, addictions, and related issues.
- Knowledge of community resources and support
- An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with client on goals/priorities.
- Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills
- Good oral and written communication skills and computer literacy, including MS word and Excel.
- Counselling, responding and assessment skills.
- Creativity and flexibility to approaches.
- Ability to work effectively as part of a team and interact with workers from a variety of agencies.
- Understanding of cultural competency and framework
- Possession of a valid driver’s license, and a reliable vehicle in order to travel is required.
Work Environment:
- Staff will be required to work in client’s own homes and may therefore be exposed to secondhand smoke.
- Occasional evening and weekend hours may be required.
- Frequent travel will be required using own vehicle.
