Case Manager - COAST (Contract) (64-25)

The Community Outreach and Support Team (COAST) provides intensive case management and assertive community treatment for people with serious mental illness.The Case Manager with the COAST team is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with mental illness. This team member has clinical skills and experiences to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services with clients with severe and persistent mental illness under the clinical supervision of the team leader.
