Caregiver Support and Community Navigator
Full-time contract position for 6 months with the possibility of extension, 35 hours per week, Monday to Friday 8:30 AM - 4:30 PM with occasional work outside of business hours. Hybrid position with some travel required within York region for home visits to complete assessment/re-assessment as required.
The Role
Reporting to the Integrated Care Manager, you will be responsible to plan and implement a defined set of community services to support in-home transitions as a core care partner, including supports for Caregivers.
You will be responsible to complete client care needs assessment (independently or with a partner agency), provide care coordination, liaise with relevant CHATS program supervisors and coordinators and serve as the primary contact for CHATS and to provide Caregiver Support and Education within the scope of the program.
Key responsibilities of the position:
- Provision of timely and comprehensive client needs assessments with a focus on safety and risk and ongoing case management.
- Complete assessment/re-assessment home visits as determined by the program protocol or as triggered by change in status.
- Provide direct service to caregivers including assessment, wellness education, training or information services, and maintain integrity of client records.
- Contribute to the delivery of program services through team building, knowledge sharing, education on and advocacy for CHATS programs and service through excellent customer service, continuous quality improvement and safety and risk management.
- Foster positive relationships with care partners, Ontario Health@Home, Home and Community Care and other referral sources to promote service awareness, increase referrals to CHATS services, identify and build service opportunities.
What you bring to the role:
- Diploma in Social Service Worker or other healthcare related field education/experience.
- Two years’ experience with seniors and/or provision of client and/or caregiver services in the community, including program development.
- Excellent skills in risk, crisis, and conflict management.
- Excellent verbal and written communication/documentation skills.
- Experience working in multi-partner initiatives is preferred.
- Proficiency in MS Word, Excel, Outlook, and data base applications (GoldCare).
- A valid Ontario drivers’ license and a vehicle are required for this position.
- Ability to accommodate a flexible work schedule including evenings and weekends.
- Current certification in First Aid and CPR.
- You will work within an integrated health care environment and possess excellent assessment, communication/documentation, case management, organizational and problem-solving skills.
