The HART HUB at 40 Oak Street is an innovative program designed to provide trauma-informed, client-centered addictions counseling and recovery support within a low-barrier supportive housing environment. Our mission is to offer safe, stable housing and comprehensive care for individuals experiencing chronic homelessness, poverty, and the lasting impacts of trauma.
We are seeking a dedicated Addictions Case Manager to deliver trauma-informed, recovery-oriented support to tenants living in our supportive housing community. This role focuses on building trusting relationships, developing individualized case plans, and coordinating resources to help clients achieve their goals related to substance use, wellness, and housing stability.

What You Will Do:
- Conduct client-centered intakes and assessments to understand substance use history, mental health, goals, readiness for change, and strengths.
- Utilize motivational interviewing, trauma-informed practices, and a compassionate care approach during all client interactions.
- Assess clients' support needs weekly based on indicators of stability (e.g., daily living skills, housing, finances, healthcare, social networks).
- Develop, implement, and regularly review individualized Case Support Plans, Housing Plans, and Coordinated Care Plans (CCPs) that integrate addiction recovery goals with housing stability and wellness.
- Maintain a dedicated caseload of clients with high and medium support needs.
- Conduct regular case management meetings to advance clients' immediate needs and mid-term goals.
- Draft clear, coordinated care plans and adjust them periodically throughout the service.
- Facilitate programming groups addressing life skills, and social isolation, with emphasis on community development.
- Coach clients in the development/maintenance of activities of daily living, self-care, budgeting, grocery shopping, and maintaining a support network.
- Provide compassionate care support, education, and connect clients to relevant community resources.
- Identify, intervene in, and de-escalate crisis situations with a focus on safety, trauma-informed practice, and compassionate care.
- Work with tenants to develop and strengthen their own crisis management skills.
- Work collaboratively with the housing workers and addictions counsellor to ensure a holistic approach, aligning addiction recovery with housing stability.
- Positively engage, encourage, and support participants to address tenancy-related issues (e.g., behavior, substance use, mental health, hygiene, social isolation) through problem-solving and accessing resources.
- Educate and empower tenants regarding their health, tenancy rights, and responsibilities.
- Work collaboratively within a multi-disciplinary team (housing workers, mental health specialists, medical professionals) to provide integrated care.
- Develop and maintain suitable community partnerships; make referrals and liaise with external programs and agencies.
- Participate in case conferences, team meetings, and community development activities that foster a supportive recovery environment.
- Accurately document all client interactions, progress notes, assessments, and statistics in the organization's database (e.g., EMHWare) in accordance with professional standards.
- Maintain strict confidentiality and demonstrate appropriate professional boundaries with clients, staff, and community partners.
- Participate in regular supervision, team meetings, and the ongoing evaluation of program development.
- Perform other duties as assigned by the Senior Manager.
What You Bring to the Team:
- Degree of Diploma in Social Services and 2 years minimum case management experience
- Strong understanding of systems that cause and support homelessness, poverty and discrimination and respect for those realities.
- Experience working with people who are actively using substances
- Extensive case management experience with a focus on supportive housing environments
- Strong understanding of the systemic issues that cause and sustain homelessness, poverty, addiction, and discrimination.
- Experience working directly with individuals from diverse racial, ethnic, and cultural backgrounds, and individuals with varying mental health and trauma experiences.
- Highly developed crisis intervention, conflict resolution, and de-escalation skills.
- Well-developed interpersonal and communication skills, with the ability to build trusting, professional relationships.
- Strong understanding of group facilitation and experience running programs.
- Ability to work productively both independently and as part of a collaborative team.
- Strong organizational skills and the ability to prioritize tasks in a dynamic environment.
- Proven ability to maintain professional boundaries and engage in constructive feedback.
- Knowledge of community-based resources and support systems.
- Highly developed written and oral communication skills.
- Strong understanding of and experience in community development practices.
- Proven ability to challenge organization, staff and tenant communities in a positive and creative way to effectively improve service.
- Literacy in Microsoft Office Suite and experience with client data management systems (e.g., EMHWare) is an asset.

Compensation and Benefits:
- For the Addictions Case Manager - HART HUB Housing Program position, Fred Victor offers an hourly rate of $29.40 per hour.
- 4 weeks of vacation to start + 1 float day
