West Durham Family Health Team

The West Durham Family Health Team (WDFHT) was established in 2007 as part of the third wave of Family Health Teams announced by the Ontario Ministry of Health. The Team was incorporated as a non-profit organization in June 2007. Since that time, we have worked towards building a collaborative, multidisciplinary team to meet patients pirmary care health care needs in the West Durham region.

Our team comprises Registered Nurses, a Registered Practical Nurse, Nurse Practitioners, Social Worker, a Registered Dietician, Occupational Therapist and a Consutling Pharmacist.

The WDFHT's mission is to enhance the quality of a patient's health and well-being through accessible and comprehensive care integrated with community resources, providing the Right Care at the Right Time in the Right Place through a patient-centred care approach.
