There are many opportunities for friendship and social interaction; however, each resident is free to choose his or her own balance of privacy and the level of social involvement that suits them best.What You Will Do:Support of the physical and mental health requirements of clientEstablish a working rapport with clients that supports their dignity, encourages personal empowerment and facilitates addressing their psychosocial, behaviour management and medical needs.Support clients with reminders related to taking their medications at the scheduled time(s) and in accordance with their physicians' instructions.Adhere to residence/program established medication system procedures, including ordering, recording and reminding clients about the administration of their medication as well as the self- medication program.Monitor and support clients with medical problems and illnesses, including cleaning areas which require immediate attention, i.e. linens, floor coverings, etc. for the comfort and support of the clients. Originally known as the Drop In Centre, Stepping Stone was founded in 1983 by a local nun, Sister Christine Leyser, to address a need for support, food and accommodation for those affected by homelessness. An emerging need for emergency overnight shelter led to the opening of Stepping Stone in 1986 in partnership with the County of Wellington.&nbsp;Although the Drop In Centre and the Stepping Stone addressed a critical need for emergency shelter, there was a growing recognition over the ensuing years that without an increase in permanent housing, homelessness would continue to grow. Consequently, in 2021 the Drop In Centre restructured under the name Stepping Stone and shifted its primary focus from providing emergency shelter services, to securing and providing permanent housing for those experiencing homelessness. A significant milestone in realizing this new mission was the opening of Grace Gardens in August 2023. With the assistance of the Federal Government and the County of Wellington, Stepping Stone purchased and renovated a local motel, transforming it into 32 permanent supportive housing units, now collectively known as Grace Gardens.&nbsp;Stepping Stone has grown rapidly over the past five years and now provides a broad array of services including outreach for those unwilling or unable to engage with the health and social service system; emergency overnight shelter; preventative and ongoing support for those requiring assistance to maintain their housing; and the provision of permanent housing through facilities such as Grace Gardens.&nbsp;The Board and Staff have shown adaptability, resilience, and dedication over the past few years of rapid expansion and are strongly committed to finding and applying innovative and creative solutions to overcoming homelessness. All Facilitator/Employment Transition Coaches are also required to travel to and work from our Scarborough and Toronto offices on a rotational basis to support program delivery.The successful candidate should possess&nbsp;a valid Ontario G driver’s licence and a reliable vehicle&nbsp;to perform the core deliverables of the position that require frequent travel, including to locations not accessible by transit.Vulnerable Sector Check:An offer of employment for this opportunity will be contingent on the incumbent providing an acceptable Vulnerable Sector Check (VSC).Start date:As soon as possibleReporting to:Program ManagerResponsibilities include:Workshop Facilitation<ul><li>Designs and delivers engaging in-person workshops through interactive activities and discussions focused on changing mindsets, goal setting, dispelling myths about work, and progressing clients towards enrolling in an employment program.</li><li>Maintains an effective working relationship with clients when presenting workshops, ensuring clients with different learning styles and needs are accommodated, including conducting knowledge checks and feedback sessions to ensure reflection, progress, and continuous attendance.</li><li>Coordinates guest speakers, training opportunities, and group mentorship events to advance clients’ learning, exposure, and networking.</li><li>Manages conflict or disruptions during workshops in a proactive, positive, and professional manner.</li><li>Facilitates direct coordination of services with employment service partners, ensuring clients successfully transition to Phase 2 of the program, which is active job search.</li><li>Follows up with each client to ensure their positive engagement in the job search program and provides support as needed.</li><li>Fosters an alumni network and facilitates follow-up workshops, including life skills sessions that support active job search and job retention topics.</li><li>Performs other relevant duties as assigned</li></ul>Employment Transition Coaching<ul><li>Conducts accurate assessments of potential clients and their eligibility for the program.</li><li>Assesses the needs of clients and identifies their barriers to program participation and employment, utilizing prescribed processes and systems.</li><li>Develops customized service plans with appropriate coaching support and employment exposure to help clients transition into active job search.</li><li>Provides one-to-one life skills coaching to clients, encouraging them to apply skills learned through workshops and progress toward their goals and employment program participation.</li><li>Coaches and motivates clients during the program and follows up during the early phases of Phase 2, which involves active job search.</li><li>Partners with the employment program to support the clients’ job search engagement and progress toward employment.</li><li>Utilizes systems and technologies provided by The Career Foundation to further support clients in achieving successful outcomes.</li><li>Performs other relevant duties as assigned.</li></ul>Outreach to Participants and Employers<ul><li>Performs effective outreach to recruit eligible participants who have been receiving Ontario Works Income Support or have been unemployed for an extended time.</li><li>Presents the program to community partners and potential participants, ensuring they understand the program objectives.</li><li>Conducts outreach and cold calls to employers to promote the ReSet program and arranges guest speaking sessions, workplace tours and exposure opportunities, mentorships, and professional networking sessions.</li><li>Develops two new employer contacts each week and eight new employer contacts each month to generate work exposure opportunities for clients of the program.</li><li>Conducts accurate assessments of participants based on program eligibility requirements and ensures all mandatory intake forms are completed.</li><li>Supports the Program Manager in preparing communications about the program, including success stories and entries for annual reports.</li><li>Performs other relevant duties as needed.</li></ul>Case Management &amp; Documentation<ul><li>Maintains accurate case notes and files in compliance with internal policies and funder requirements.</li><li>Prepares and maintains documentation on client intakes, referrals, attendance, suitability indicators, outcomes, and other required information.</li><li>Monitors clients’ progress with ongoing follow-up calls and meetings and ensures all appropriate documentation is recorded in online reporting systems.</li><li>Provides the Program Manager with documentation for supports to be purchased for program participants.</li><li>Ensures that client records are kept up-to-date and audit-ready.</li><li>Performs customer service and administrative functions, including document creation and file retrieval.</li><li>Performs other relevant duties as assigned.</li></ul>Qualifications/ Skills Required:<ul><li>Post-secondary degree or diploma in human services, social sciences, or a related field is highly preferred, or an equivalent combination of education and experience is required.</li><li>Must possess a minimum of two years of related experience in workshop facilitation, job or life skills coaching, or other professional experience that is directly transferable to the responsibilities of the position.</li><li>Confident public speaking and presentation skills to effectively engage clients during workshops and other program activities.</li><li>Excellent verbal and written English communication skills.</li><li>Valid Ontario G driver’s licence and reliable vehicle to conduct essential work-related travel.</li><li>Experience with community outreach, recruitment, and/or developing relationships with employers considered strong assets.</li><li>High emotional intelligence, demonstrating patience, empathy, and connectivity when working with clients of various experiences, needs, abilities, and cultural or social backgrounds.</li><li>Demonstrated track record in a results-driven environment with experience meeting targets.</li><li>Exceptional customer service skills and the ability to uphold the organization’s service standards, policies, and core values.</li><li>Knowledge of current labour market trends, job search techniques, and employment research methods is considered an asset.</li><li>Professional and courteous approach to client service with a high level of respect for the diversity of the people served.</li><li>Ability to maintain sensitive information and exhibit tact, diplomacy, and good judgment, with high standards of ethics and confidentiality.</li><li>Advanced proficiency in Microsoft Office Suite (Word, PowerPoint, Excel) and Outlook 365 applications, including Teams and SharePoint.
