Eligibility includes those designated as Alternative Level of Care (ALC) or at risk of being designated, and who need around-the-clock support to remain safely at home. Referrals are managed through Centralized Intake, with sources typically being hospitals in the North Simcoe Muskoka (NSM) region. Once referred, potential residents undergo an intake process by LOFT, which includes a facility tour and acceptance of the housing offer. Our facility possesses 27 beds, comprising 24 permanent and 3 transitional options, offering both private and shared accommodations. Additionally, we provide Hub and Spoke services, which are community-based support designed for those who, while able to live at home, need ongoing care with personal supports available 24/7. This system is supported through our existing Assisted Living buildings, ensuring that clients receive crucial support while remaining in their own homes. For those under 55 requiring significant assistance with activities of daily living (ADLs), we consider cases on an individual basis, ensuring that every resident’s unique needs are met with tailored care.What You Will Do<ul><li>Support of the physical and mental health requirements of client<ul><li>Establish a working rapport with clients that supports their dignity, encourages personal empowerment and facilitates addressing their psychosocial, behaviour management and medical needs.</li><li>Support clients with reminders related to taking their medications at the scheduled time(s) and in accordance with their physicians' instructions.</li><li>Adhere to residence/program established medication system procedures, including ordering, recording and reminding clients about the administration of their medication as well as the self- medication program.</li><li>Monitor and support clients with medical problems and illnesses, including cleaning areas which require immediate attention, i.e. linens, floor coverings, etc. for the comfort and support of the clients. You will complete a comprehensive behavioral assessment and an evidence-based Behavioral Support Plan. You will assist in the creation and coordination of a support network through establishing linkages with other community agencies and practitioners to coordinate care for the clients. In addition, you will provide&nbsp; direct service to support the client, family members and/or care-givers to mitigate escalating responsive behaviors. You will also liaise, collaborate and communicate with all members of the Behavioral Support Services team and other partners within the circle of care.&nbsp; The Behavioural Support Services - Mobile Support Team provides direct care, education and support to older adults and their care partners, who are demonstrating responsive behaviours/personal expressions that may be associated with various types of dementia, mental health, substance use or other neurological conditions living within the community within North Toronto, York Region and South Simcoe. The team completes a comprehensive behaviour support assessment and creates a person-centered Behavioural Support Plan. Transition support is provided to those individuals who a transitioning to another care destination or to another care team. Case managers offer support and provide care coordination, crisis intervention, system navigation and client advocacy. Mental, physical and spiritual health are enhanced by social interaction provided through Dunn’s life enrichment program. Dunn services are offered to adults 59 and older; program size is 60 individuals. Dunn’s Mobile Hub and Spoke program also provides PSW support to an additional 26 clients at risk for ALC, living in the community. These include small group living arrangements for those requiring minimal support, individual and shared accommodations, and staffed congregate settings for individuals needing daily support. Within that continuum, we also serve people with current involvement in the criminal justice system or those with several comorbid health issues. What You Will Do Through a team oriented professional approach, the position provides support to the client to increase their independence, dignity and self-respect, in a safe and secure environment by:<ul>	<li>Building a rapport with each individual and providing coaching or mentoring to enable each in meeting the daily requirements of their housing environment and a healthy lifestyle; directly assisting with their daily living activities, particularly in the acquisition of life skills in such areas as hygiene, medication management, cooking, meal planning, cleaning, shopping and budgeting;
