JOB PURPOSE 
As a key member of the Student Wellbeing team, the Social Worker will work with students and 
their families within our Middle School division, with the objective of optimizing their overall 
wellbeing at school. Reporting to the Associate Head, Wellbeing, the Social Worker will have a 
comprehensive understanding of the psychosocial needs of young children, and will support 
students and families in addressing the social, behavioural, and emotional factors which impact a 
student's overall school experience. 
 
This role follows a 10âmonth work schedule aligned with the academic year, working from 
late August to late June each year and is off during the summer. 
 
Given the dynamic and collaborative nature of our school environment, this role is an on-campus 
position requiring to be on-campus five days a week. Candidates should be prepared to 
contribute actively to this in-person, community-focused environment. 
 
KEY RESPONSIBILITIES 

â Consultation with students, parents and faculty; provide guidance and assistance in 
support of the psychosocial health and well-being of students 

â Provide individual, group and family counselling 
â Conduct assessments of students’ needs 
â Support the enhancement of partnerships with community agencies and outside 

professionals to ensure optimal support for students and the school community; liaise 
with community agencies as required 

â Provide crisis intervention services 
â Develop and facilitate groups for students 
â Establish a visible and approachable presence in the Middle School community 
â Participate in team meetings and regular planning regarding individual case management 

and school-wide programs 
â Manage and maintain student records and caseload ensuring services are integrated and 

seamless 
â Understand, and practice in accordance with, federal, provincial, and local laws, statutes, 

and/or policies that relate to students and families; such as, child protection/child abuse,

QUALIFICATIONS AND SKILLS 

â Masters degree in Social Work (or equivalent) and membership in good standing with the 
Ontario College of Social Workers 

â Minimum 5 years’ experience, including experience working in an educational and clinical 
settings with children 

â Knowledge of the Social Work and Social Service Work Act, The Child and Family 
Services Act, Education Act, Health Care Consent Act, and Mental Health Act 

â Ability to identify, assess and respond to sensitive and confidential student and 
community issues

We have a holistic approach to mental health care and operate within an anti-racism/anti-oppression and resisting anti-Black racism frameworks.These frameworks focus on providing opportunities to respond to systemic barriers, advocate for the inclusion of traditionally and historically silenced voices at decision-making tables, and strive for change at individual, organizational and systemic levels.Job Summary:Across Boundaries is recruiting for a permanent full time Case Manager to provide services to those experiencing serious mental illness/addictions and are involved with the criminal justice system. The workers will report directly to the Program Manager and work collaboratively within and across teams to provide prompt and appropriate access to services to individuals from racialized communities who are experiencing mental health problems and substance use challenges. The workers will provide both intensive and non-intensive services depending on serviceuser needs and will have an average caseload of 20-25. Normal Hours for full-time employees consist of five (5) shifts or eight (8) hours per day, Monday to Friday, and Saturdays on a rotating basis, inclusive of a thirty (30) minutes unpaid lunch break (entitled under the ESA) and an additional two (fifteen (15) minute unpaid rest breaks. Normal hours of work will generally be scheduled between 8:00 am and 8:00 pm. JOB DESCRIPTION: 

1. To liaise with community stakeholders through engagement with local groups in
the South Etobicoke area.

2. Recruit, train, hire, orient, supervise, coach and debrief Case Managers on a daily
basis.

3. Supervise students and volunteers and staff from in your related dept.
4. To ensure that all deliverables are achieved as required by our funders as per our

program Work Plans.
5. Assist with the completion of funding reports/applications, and research

initiatives as directed.
6. To maintain and submit monthly program statistics.
7. Liaise with funders and government representatives when required.
8. Complete annual staff performances including student placement evaluations;
9. Weekly and monthly tabulation and reporting of statistics internally and

externally (to funders as required) including housed and discharged client
documentation, monthly TGRIP data entry and six month reports;

10. Design and implementation of on-going follow-up services for clients after they
are housed;

11. Advocacy with landlords and other housing providers;
12. Liaison with other housing and supports agencies;


13. Development and Submission of reports, proposals and other government 
requirements as requested; 

14. Distribution of TTC fare and submissions related documentation to clients; 
15. Complete annual staff performances including student placement evaluations; 
16. Development of innovative strategies to assist ‘hard to house’ clients; 
17. Oversee case management and provide supportive counseling; 
18. Ability to resolve and de-escalate crisis situations; 
19. Conflict mediation relevant to housing maintenance 
20. Provide Senior Management Team with regular feedback about the evolution, 

program needs. 
21. Submission of Furniture Bank requests and Rental Supplement request in a 

timely manner; 
22. Daily oversight of staff attendance and DayForce entries; 
23. To maintain the confidentiality of all matters related to Human Resources. 


Administrative: 
 
1. To maintain accurate record keeping i.e. client files, statistical reports. 
2. To assist in training and supervision of students and volunteers as time allows. 
3. Assist Director with programs statistics for reporting to funder 
4. To assist in other duties as required from time to time.

1. Minimum of 2 years supervisory/management experience 
2. Minimum of College Diploma or University Degree in social services. 
3. Minimum of 3 years’ experience in social services or a related field. 
4. Experience in program development and program evaluation. 
5. Some knowledge of program and process evaluation and an interest in 

research 
6. Knowledge of community reintegration barriers for formerly incarcerated 

persons, provincial court/correctional processes, housing and homelessness 
in a local context 

7. Demonstrated experience working with a diverse ethno-racial and multi-
lingual client group. 

8. Commitment to the principles of anti-racism, anti-sexism, and to challenge 
other forms of oppression. 

9. Strong verbal and written communication skills, administrative skills, and a 
willingness to work as part of a team. 

10. Recent criminal record check
