As a key member of the Supportive Care Department, the Supportive Care Coordinator is primarily responsible for leading and delivering grief and bereavement services at Oak Ridges Hospice.Reporting to the Supportive Care Department Lead, the Coordinator will focus on both resident/family support and community-based programming. The role emphasizes improving accessibility to grief services, developing tailored programs, strengthening grief volunteer engagement, and expanding outreach through innovative approaches such as community partnerships, events, and digital resources. The Coordinator will provide grief support consistent with the Adult Bereavement Care Pyramid (Irish Hospice Foundation), focusing on Level One and Level Two support<ul><li>Grief and bereavement support at Level One and Level Two of the Adult Bereavement Care Pyramid, including psychoeducation, resources, early intervention, group programs, workshops, and memorial events.</li><li>Conduct comprehensive grief and bereavement intakes and assessments to determine appropriate support pathways.</li><li>Develop, implement, and evaluate grief support programs in collaboration with the Supportive Care Department Lead.</li><li>Improve accessibility to grief and bereavement services for hospice residents, families, and the broader community.</li><li>Coordinate and collaborate with the Supportive Care Department Lead to align grief programming with overall departmental goals.</li><li>Oversee, mentor, and provide ongoing support to Grief &amp; Bereavement (G&amp;B) volunteers in collaboration with the Volunteer Coordinator.</li><li>Enhance outreach through digital initiatives such as a dedicated grief blog, podcast, or webinar series.</li><li>Partner with community organizations, schools, and healthcare providers to expand awareness and access to grief supports.</li><li>Maintain accurate documentation of client interactions and program outcomes.</li><li>Assisting with special events as required.</li><li>Post-secondary degree in&nbsp;Social Work, Counselling, Psychology, Spiritual Care, or a related field.</li><li>Specialized education/training in grief and bereavement support (e.g., certificate programs, ADEC, Canadian Grief Alliance standards, or equivalent) is preferred.</li><li>Knowledge of the Adult Bereavement Care Pyramid and its application in community-based services.</li><li>Demonstrated experience providing grief support in one-to-one and group settings at Level One and Two of the Bereavement Care Pyramid.</li><li>Experience in program development and coordination, including grief groups, workshops, or community-based initiatives.</li><li>Experience working with or coordinating volunteers in supportive or bereavement roles.</li><li>Prior experience in hospice, palliative care, community health, or supportive care is an asset.</li><li>Excellent interpersonal and communication skills, including group facilitation.</li><li>Understanding of grief literacy and ability to promote bereavement awareness within the community.</li><li>Ability to demonstrate compassion, empathy, integrity, respect, courtesy and a caring attitude in all interactions. </li><li>Pre-existing knowledge of community health and social service resources an asset. </li><li>Computer literacy. </li><li>Current and original copy of a satisfactory Criminal Records Check, Vulnerable Sector Search and Child Abuse Registry Check is required upon the start of employment.</li></ul>Current and original copy of a satisfactory Criminal Records and Vulnerable Sector Check is required upon the start of employment.Oak Ridges Hospice recognizes the importance of immunization to protect our residents, team members and visitors from COVID-19. Sloane’s House is where families of children, aged up to twenty one (21) years, with complex 
health needs will come to join our family of caring professionals; a supportive community of 
parents and caregivers. Located in Whitby, it will be available for those living across Durham 
Region and in the surrounding underserved communities. A rejuvenating place for respite stays 
offering engaging programing for the child or young person, while parents and other caregivers 
take a much-needed break. And for those children and youth who are on their end-of-life 
journey, a place where specialized care will be provided in an environment where quality of life 
will be the focus for however many days remain. 
 
With an estimated 1,900 children with medically complex needs in Durham Region and 
surrounding communities, 800 in Durham alone, requiring paediatric medical respite and 
hospice care, the need for this Children's Hospice Is clear.

Currently, we are looking to fill three board positions with individuals who have experience in 
the following areas: 
 

• Events &amp; Fundraising – Experience developing, planning &amp; executing community events, 
vendor sourcing and event budget management. Event management certificate an 
asset. 

• Finance &amp; Audit – A strong business acumen for budget creation, management and 
oversight, non-profit standards for tax and audit. 

• Health Care – A background in a relevant area of the health care sector, particularly with 
insight into policy and operations relevant to respite and/or hospice. 

 
Time Commitment: 

• The board meets as a whole 6-8 times per year (except during the summer). 

• Each director available to serve on at least one Board committee that meets monthly 
(except the summer), and should actively participate in annual fundraising and other 
special events. 

• Directors are expected to serve, at least, one 2-year term. They can be reappointed for 
up to two more terms.
