The Site support workers provide social recreational and educational groups/support to tenants and staff within the homes, while the Case Manager provides more individualized supports for those tenants requiring it.&nbsp; &nbsp; Toronto West Integrated Service Team - TWIST provides individualized support to service users living with a serious mental illness in West Toronto. The multi-disciplinary team consists of eight (8) Mental Health Case Managers, two (2) Addiction Specialist Case Managers, two (2) Acquired Brain Injury Case Managers, one (1) Psychogeriatric Case Manager, two (2) Dual-Diagnosis Case Managers, and one (1) Community Supportive Housing Worker.
