Baldwin Housing and Community Support Program is seeking a Community Support Worker - 6 Month Contract - to provide instrumental support services to individuals aged 16 or older who have a serious mental illness/concurrent disorder. Services will be provided within a framework of a client-centered rehabilitation model. The community support worker will work collaboratively with the team and other mental health service providers to increase the functional abilities and independent living of residents of the Baldwin Housing and Community Support Program. The Community Support Worker will perform a continuum of functions, including developing support plans with clients, client, and systems advocacy, symptom management, life skills teaching, supportive counselling, family support, and crisis support. The Community Support Worker provides support within our residential setting and in the Community with individuals living independently. The community support worker may provide on-call support and, at times, works flexible hours.Qualifications:Candidates must have a college diploma/university degree in social service worker or social work with experience in the mental health field and/or addictions and housing. The successful individual will embed this guiding principle into their work.About the Position: The Resource Worker, Dual Diagnosis provides support to clients living with a dual diagnosis (co-occurring developmental disability and mental illness), as well as clients living with symptoms of a mental illness. The Resource Worker also works with clients who have dual diagnosis and justice system involvement. She/he is responsible for engaging with each client to provide social and community supports to help them maintain and thrive in their home. This will include developing goal plans, providing service coordination, coaching, skills teaching, safety planning and linking/referral supports aimed at assisting each client to achieve his/her goals.Resource Worker must collaborate with clients, client families/natural support networks and agencies/services in the mental health, justice, and developmental services sectors to effectively fulfill their role.Primary Responsibilities:<ul><li>Reviewing the referral information of applicants to the program.</li><li>Coordinating and participating in screening interviews with applicants and their respective referral sources.</li><li>Determining eligibility for the program and confirming acceptance of new clients into the program. Completing a comprehensive assessment of the nature of each client’s unique goals and needs through collaborative engagement with each client and their respective support networks.</li><li>Assist clients in the development and implementation of a care plan tailored to the client’s needs and issues designed to improve their quality of life</li><li>Act as a liaison and client advocate among those involved with the client’s care including family, other caregivers, medical and other multi-disciplinary providers.</li><li>Conduct assessments and provide timely and accurate record keeping and reports as required.</li><li>Provide supportive counselling or referral and linkages to health care, programs, or service providers.</li><li>Provide crisis management/intervention and assist in the development of crisis plans for clients.</li><li>Provide flexible and intensive one-to-one individual support and follow-up to the clients and assist them to develop and reach their recovery goals.</li><li>Assess clients for the need for other services, including nursing, psychiatric, medical, harm reduction, peer support and addictions assessments and treatment.</li><li>Assist individuals to access a range of services in the broader community.</li><li>Accompany clients to relevant appointments when needed. Advocate on behalf of homeless individuals in accessing appropriate services such as health care and financial supports.</li><li>Work cooperatively with other service providers to better meet the needs of clients.</li></ul>The Successful Candidate Requires:<ul><li>Diploma in Developmental Services Worker, Counselling or Social/Human Services from a recognized University or College.</li><li>One year experience supporting people with developmental disability, mental illness, or dual diagnosis in a Developmental Services Organization.</li><li>Sound theoretical and practical knowledge of co-occurring developmental disability and mental illnesses.</li><li>Demonstrated ability to use problem-solving techniques to identify and assess client needs and develop appropriate intervention strategies.</li><li>Knowledge of principles of case management theory and practice.</li><li>Service coordination experience including initial and ongoing assessments, goal setting, short and long-term care plan development, crisis intervention, progress monitoring, and individual financial management .</li><li>Proficient in MS Office.</li></ul>Preferred Qualifications:<ul><li>Experience in community services and resources relevant to the specific client population.</li><li>Knowledge of Anti-Racism Anti-Oppression framework as well as an ability to articulate and implement Recovery Philosophies.</li><li>Experience in working within a multidisciplinary team.</li><li>Experience with group facilitation.</li></ul>Working Conditions:<ul><li>Comfortable working with a broad range of client groups. The Housing Support Worker will assist with the securing of appropriate housing within the community, liaise with landlords, and advocate on behalf of clients. Support and instruction will be provided to individuals as needed, to facilitate learning and maintaining activities of daily living which endeavour to prevent future involvement with the criminal justice system and retain housing. The Housing Support Worker will assist clients to develop their strengths, promote wellness and housing stability, and improve their quality of life.QualificationsCandidates must have a university degree/college diploma in social work or social service worker with experience in the mental health field and/or addictions and housing.
