About the positionThe Regional Support &amp; Education Coordinator, Community Services is responsible for community engagement and service delivery throughout the Yukon. In this role you work both independently and as a part of a team, and you are responsible for establishing and delivering the Society’s First Link® programs and services within the territory. Responsibilities include developing and maintaining referral pathways to connect clients to support services and resources, and delivering standardized workshops. As this role is fully remote you must be self-motivated and able to work independently. care, biomechanical assessments, custom orthotic casting and dispensing, wound care, measuring for 
compression stockings, and conducting procedures such as PNA, TNA and soft tissue lesion excisions 

 • Works closely with staff to provide foot care within a broader multi-disciplinary team; works particularly 
closely with the diabetes nurse and diabetes dietitian 

 • Builds trust and rapport with clients to promote effective service use 

 • Provides health education, harm reduction education, and advocacy to HART Hub clients 

 • May provide services at designated outreach locations in the community and in the mobile health unit

While the proportion of time allocated to different functions may vary according to service needs, the role encompasses a combination of direct care, health promotion and education, advocacy and community development.RESPONSIBILITIES<ul><li>Co-facilitates group diabetes education sessions for adults with type 2 diabetes and those</li><li>who have pre-diabetes by disseminating appropriate nutrition education that is consistent with</li><li>clinical best practices for prevention and management of diabetes, ensuring a safe and</li><li>barrier-free environment for learning and sharing of experiences, and ensuring client needs are met in a relevant way.</li><li>Conducts nutrition assessments, including taking a complete medical and social history and works with clients to develop nutrition plans, taking into consideration such factors as psychosocial, economic, functional and family status</li><li>Provides individual nutritional counseling to clients with type 2 diabetes and pre-diabetes utilizing a self-management approach, thus working collaboratively with clients to work towards client identified goals.</li><li>Uses a wide range of modalities for teaching/counseling to enable clients to acquire the knowledge, skills, and confidence needed to effectively manage diabetes.</li><li>Ensures continuity of care by maintaining up-to date medical records, communication with the client’s primary care provider and other service providers, as required..</li><li>Provides health related equipment or supplies to clients and advises on proper use.</li><li>Identifies trends or issues related to the health status of priority groups living with diabetes and uses this information in planning and advocacy work.</li><li>Provide care delivery at any of the office sites as program needs dictates Other duties as assigned.</li></ul>QUALIFICATIONS, EXPERIENCE &amp; SKILLS<ul><li>Undergraduate degree in food and nutrition from a recognized university or equivalent.&nbsp;</li><li>Completion of a Canadian Dietetic Association accredited Dietetic Internship</li><li>.Registered with the College of Dietitians of Ontario in good standing</li><li>A minimum two years experience working as a diabetes educator or working in a community-based health promotion setting working in a similar position.</li><li>Certified Diabetes Educator (CDE) is an asset or willingness to acquire.</li><li>Demonstrated experience in health promotion program planning, development, implementation, and evaluation.</li><li>Demonstrated experience in facilitating groups and providing individual counseling using a client centered approach.</li><li>Demonstrated commitment to best practices, ongoing learning, and client centered care.</li><li>Experience working with communities and individuals who face barriers accessing traditional health care institutions due to poverty, discrimination, mental health and substance use issues.</li><li>An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.</li><li>Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.</li><li>Experience working with confidential and time sensitive documents and information.</li><li>Excellent interpersonal skills and the ability to multitask in a fast paced environment.</li><li>Experience working in a value based non-profit or social services organization.</li><li>Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.</li><li>Demonstrated proficiency in use of computers and various computer software applications, such as Electronic Medical Record (EMR).</li><li>The ability to speak languages relevant to the residents of the organization’s community is an asset.</li><li>Holds a valid Ontario driver’s license, and access to a reliable vehicle with insurance for responsive care delivery at any care delivery location.</li></ul>Candidates who self-identify as being from a marginalized community, including people with lived/living experience of drug use and/or houselessness, Indigenous peoples, racialized persons, persons with disabilities, newcomers, and 2SLGBTQIA+ persons, are encouraged to apply and will be prioritized.
