Our Agency is committed to taking the journey together and supporting those with whom we work to grow, thrive, and contribute to society.About the Nurse Practitioner Team:&nbsp;The Nurse Practitioner team at STC provides low-barrier addictions medicine care to individuals with substance use disorders in a variety of settings, including virtually through the Digital Front Door program, within Rapid Access Addiction Medicine (RAAM) Clinics (Guelph and Fergus), in-home withdrawal support alongside the Community Withdrawal Support Services (CWSS) team, and in a residential withdrawal management setting through WRHN at Midtown (formerly Grand River Withdrawal Management).&nbsp;The successful candidate has a passion for Addictions Medicine and embraces a harm-reduction approach to care. They will be joining a team of passionate Nurse Practitioners, Counsellors and Peer Workers working across Waterloo-Wellington. Opportunities for professional development, addictions medicine consultation, and supervision will be provided.Why Work for Us?<ul><li>Competitive Wages - Stonehenge compensates NP staff at a primary care rate</li><li>Enhanced vacation entitlement</li><li>Professional and career development opportunities&nbsp;</li><li>Enhanced wages available for hours worked at WRHN at Midtown</li></ul>Current Vacancies - Location and Hours of Work Information:<ul><li>Guelph and Fergus RAAM locations (hours available Monday to Friday)</li><li>WRHN at Midtown hours available Monday to Friday; weekend hours also available)</li><li>All hours scheduled 8:30am-4:30pm</li><li>Remote work available through the Digital Front Door (Monday to Friday)</li></ul>Qualifications:<ul><li>Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred</li><li>Current registration with the College of Nurses of Ontario as an RN(EC) required</li><li>Membership with NPAO or RNAO an asset</li><li>Evidence of professional practice liability insurance required (minimum $5M, per professional requirements)</li><li>Completion of suboxone and addiction medicine education</li><li>Certified in prescribing narcotics and controlled substances</li><li>Demonstrated experience in Community and/or Primary Care Setting</li><li>Current Ontario Driver’s License, with access to reliable transportation</li><li>Standard First Aid/CPR Level C and AED</li><li>Required to meet vaccination and occupational health requirements of working within partner agencies (example - COVID-19, other proof of immunization)</li><li>Vulnerable Sector check required; Additional security clearance may be required depending on work location</li><li>Willingness to travel within Wellington County (RAAM Work)</li><li>Demonstrated experience in mental health and addictions preferred</li><li>Lived experience with addictions and/or mental health an asset</li></ul>Role Overview:<ul><li>Perform comprehensive care using advanced health assessment, physical examination, therapeutic management, consultation, collaboration and referrals in accordance with the Ontario College of Nurses Practice Standard: Nurse Practitioner (2018)</li><li>Assess and manage patients with addictions, who have co-existing health conditions including but not limited to mental health conditions, blood borne diseases and infections</li><li>Conduct advanced health assessment to identify clients’ emotional, cognitive and behavioral states, as well as level of anxiety, crisis states, indices of aggression, self-harm, suicide, risk to others, competency to care for self, and signs of substance abuse, addiction, and withdrawal</li><li>Utilize evidence-based interventions, including withdrawal and other medication management, to meet the needs of a diverse range of clients with substance use disorders</li><li>Provide care to persons experiencing substance use disorders and / or mental health conditions that is recovery oriented, trauma-informed and use principles of harm reduction and address social determinants of health</li><li>Act as a resource providing substance use related clinical advice for health and other professionals, community groups and clients and their families</li></ul>Required Skills:<ul><li>Demonstrated knowledge of the Mental Health Act, the Health Care Consent Act and other relevant legislation</li><li>Excellent verbal and written communication skills</li><li>Exhibit an ability to be open and non-judgmental</li><li>Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction</li><li>Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills</li><li>Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook</li><li>Prior experience and knowledge of electronic medical record</li></ul>Applicants are encouraged to apply and note their preference for hours/location and available days.We look forward to meeting you!Stonehenge Therapeutic Community accommodates the needs of applicants throughout the recruitment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) and Human Rights legislation.
