The program includes many components including, career development and life skills, academic upgrading and computer training, and a college diploma program. There are also an internship component and access to sustainable employment opportunities.&nbsp; Homeward Bound also offers on-site childcare, family support, and an after-4 program for school aged children.&nbsp;What You Will Do<ul><li>Uses professional case management practices and a variety of clinical modalities to perform case work duties which include assessment, goal setting and the development of action plans with clients, ongoing intervention, re-assessment, case termination, and follow up.</li><li>Coordinates client participation in all aspects of the Homeward Bound program including, liaising with Community Colleges and academic upgrading providers to ensure participant enrolment and full participation in programs and on-the-job training experiences such as field placements and internships.</li><li>Ensures that all clients prioritize meeting program goals and milestones.</li><li>Responsible for ongoing liaising and coordination with Toronto Children Services and Toronto Employment and Social Services.</li><li>Provides instrumental help to assist the women to resolve functional problems of daily living, to provide advocacy, and to facilitate access to appropriate support services such as health care, budget planning/financial management, and housing help. </li><li>Identifies barriers to learning and refers participants to the appropriate source of support.</li><li>Responds to emergencies and provides crisis intervention in addition to maintaining a caseload.</li><li>Maintains client records in a centralized data base in accordance with program policy.</li><li>Collects statistical data according to central and unit policies.</li></ul>What You Bring to the Team<ul><li>Master of Social Work or Master’s Degree in related field.
