Reporting to the Program Manager, you will create individualized care plans, provide post-diagnosis support, and coordinate the Halton FASD Assessment and Diagnostic Clinic, including referrals, interviews, documentation, and follow-up.You will collaborate with caregivers, educators, and service providers across sectors to build capacity, share strategies, and connect families to appropriate services. The role also involves providing training to the community, supporting FASD programs, and helping families with complex needs access coordinated service planning and transitions to adult services.This position contributes to broader system improvement by promoting FASD awareness, identifying service gaps, and engaging diverse cultural communities.
