TAIBU has grown its capacity in providing social work, therapeutic and case management services to the community over the past few years. The growth was as a result of the increase in demand for services from the community. It has now become necessary to streamline and coordinate the mental health and case management support services by social workers at TAIBU. The Team Leader is responsible for coordination, support and supervision of the Social Workers at TAIBU that are part of the primary healthcare service, the diabetes education program, the Scarborough perinatal support program, the enhanced youth outreach worker program. The Team Leader is responsible to coordinate the seamless flow of access to services and day-to-day delivery of high-quality services that respond to the cultural needs of clients. The Team Leader collaborates closely with team members, internal and external partners in organizing the delivery of culturally appropriate social work, counselling and case management services.

SPECIFIC TASKS

  • Coordinate the access of clients to the various social work, counselling and case management services
  • Monitor and manage wait lists for social work, counselling and case management services related to the services identified above (ie. primary healthcare service, the diabetes education program, the Scarborough perinatal support program, the enhanced youth outreach worker program)
  • Support social workers by providing supervision, troubleshooting, case discussions etc.. Plan, implement and monitor services that respond to client and communities’ needs, and meet internal and external performance targets.
  • Conduct ongoing mental health and wrap around services needs in the community – in particular within the Black communities
  • Plan, develop, implement and evaluate direct mental health and wrap around services
  • Provide support to existing and emerging community programs and services in providing system navigation as well as short term and crisis mental health case management and counselling interventions (one-on-one or group interventions)
  • Participate in the development and implementation of policies, protocols and workflows related to social work, counselling, case management and mental health support services.
  • Coordinate the placement of social work students in the identified programs (see above) as required.
  • Prepare and present information and reports on social work, counselling and case management activities.
  • Develop and maintain external networks and partnerships related to mental health and case management work
  • Liaise as necessary with the external clinical supervisor

Administration:

  • Participate in Centre and Board committees, work groups, projects, and chair and or take minutes at such meetings as may be required.
  • Develop a workplan in conjunction with your supervisor, identifying actions plans, learning goals and professional development needs.
  • Submit program statistics and reports on a timely basis.
  • Contribute to the vision, mission, values, principles and strategic directions of the Centre.
  • Carry out other duties as may be assigned from time to time.

Required Skills, Knowledge & Abilities

  • Master’s degree in health administration or other relevant discipline an asset
  • Three to five years of management experience in an primary care setting, preferably within a Community Health Centre or other community-based health service organization.
  • Experience working with socially and clinically complex populations
  • Strong supervisory skills, including the ability to work collaboratively with, coach and inspire teams of professionals and students
  • Knowledge and experience in staff supervision, program development, implementation and monitoring, quality improvement, financial management, project management, and policy development
  • Excellent communication skills (written and verbal) and group facilitation skills
  • Proven leadership, consultation and collaborative skills
  • Ability to work evenings and occasional weekends
  • Demonstrated ability to work independently and to be an effective member within an interdisciplinary team.
  • Demonstrated ability to manage workload with simultaneous deadlines, determine priorities and meet deadlines.
  • Proficiency in the use of computer and various software applications including word processing, spreadsheets, presentations and email
  • Proficiency in French is considered as a strong asset

OCCUPATIONAL HEALTH & SAFETY:

As a member of staff at TAIBU Community Health Centre, you have a direct responsibility for Health and Safety as an essential part of your job and must work in compliance with the Occupational Health and Safety Act & Regulations.
You are required to:
o Wear/use protective equipment (where required)
o Report hazardous situations to your supervisor
o Report immediately all incidents/accidents to your supervisor
o Use/operate equipment in a safe manner
o Refrain from pranks or contest

Compensation:
$68,475 – 82,500
