Service Coordinator – Urgent Response Services
LOCATION: Etobicoke, ON; North York, ON
POSITION TYPE: Fixed Term Full-time (18-month contract, 37.5 hours/week)

SALARY RANGE: $64,521 - $71,419

Lumenus Community Services is looking to hire a Service Coordinator as part of its community based Autism – Urgent Response Service team. This team will support children and youth with autism and their families with an identified urgent need to prevent further escalation of risk or harm to a child or youth, other people or property based on the identification of key high-risk factors.
As part of the Lumenus team, the Service Coordinator will be part of a diverse, interdisciplinary team, collaborating with other community partners to develop sustainable and equitable relations with community agencies in underrepresented areas and respond to the needs of service users in OAP Urgent Response Services.
KEY RESPONSIBILITIES:
- Perform intake and assessments and develop a working plan to support clients with complex needs
- Perform system and service coordination
- Support priority for urgent client cases
- Contact child/youth and family obtain consents, initiate service and gather any additional information from multiple sources and with clients through in person meetings, on the telephone and virtually
- Utilize a systems/ecological perspective to ensure a holistic view of the client’s strengths and needs
- Express warmth and compassion in the planning and implementation of community outreach activities through the development of collaborative partnerships with other services such as schools, agencies, child welfare, and ethno-specific organizations and communities
- Complete assessment/intake with child/youth and family to understand goals and additional resources that would be benefit the family.
- Collaborate with family and stakeholders to determine what services are requires and available
- Process referral forms submitted by lead agency and other stakeholders; follow up for any additional information, as necessary
- Engage and support promotion of Lumenus Services specifically Autism and Developmental Services, and Mental Health Services within the community by attending community fairs, delivering workshops and supporting social media initiatives
- Connect with other service navigators to share resources and best practices
- Use a Strength based anti-racism and anti-oppression philosophy of service provision
- Act as an educator/spokesperson/advocate on behalf of clients and families to funders, stakeholders, intersecting sectors, partners, and others
- Provide information to clients, caregivers, service providers and staff on the availability and appropriateness of community services and funding support
- Participate in service coordination group meetings, chairing case conferences, and community resources and meetings as required, as well as engage in community outreach to develop a collaborative approach to service delivery
- Develop positive relationships with community groups, partner agencies and resources in the larger community
- Produce client file documents that track the life of the case from entry to discharge (assessments, treatment plan and treatment reviews, and progress/contact notes)
- Ensure the client record is kept up to date and meets accreditation and best practice guidelines within the policy and procedures of the agency
- Maintain up to date client record as per agency expectation identified in service flows and through agency policy and procedures
- Utilizing agency report/note templates, ensure that written reports meet accreditation and best practice standards including producing reports at designated intervals
- Write assessment and review reports, representing the client’s view/voice as well as the multi-disciplinary team’s perspective.
- Complete internal file audit when closing client services
- Review reports with clients to ensure clients’ participation in the process
- Ensure consent to treatment is active and consent to share information with other parties are kept up to date as needed
- Ensures entries into the client data base tracks direct and indirect hours so that the evaluation of resources to outputs and outcomes can be conducted
- Maintain documentation in accordance with Legislative and Accreditation standards
EXPERIENCE/SKILLS:
- Bachelor Degree in either Social/Health Sciences, or Bachelor in CYW or Bachelor in Social Work
- 1 years of related experience working with children and youth with developmental disabilities and their families, with a focus on community engagement.
- Proven group facilitation skills, preferably utilizing Evidence Based Practices
- Proven assessment and advocacy skills
- Suicide risk assessment and intervention skills
- Ability to develop collaborative working relationships with other staff ability to connect and build rapport with adolescents
- Ability to work with and integrate knowledge from other professional staff, i.e. psychologists, psychiatrists, into the overall treatment plan
- Sound knowledge and understanding of child and adolescent development
- Knowledge of community resources and agency programs
- Ability to demonstrate and work through a lens of sensitivity to diversity related to race, religion, culture, cognitive functioning, poverty, sexual orientation and gender identity, etc.
- Ability to identify clinical learning needs and seek consultation and support through supervision
- Advanced communication & interpersonal skills; ability to work in a multi-disciplinary environment, exercising professional judgement in conflictual and stressful situations
- Well-developed writing skills with a strong record in completing documentation properly and in a timely manner
- Excellent organizational and administrative skills
ADDITIONAL REQUIREMENTS:
- Satisfactory clearance under the Vulnerable Sector Screening process
- Availability to work evenings
