You’ll provide trauma-informed mental health care to individuals living with concussion/ brain injury, helping them rebuild confidence, stability, and a sense of direction following injury.

Job Description

Registered Social Worker - Return to Work Project

Reports To: Chief Executive Officer and/or designate

Professional Designation: Registered Social Worker or Registered Psychotherapist

Term: One Year Contract (Full-Time) with possibility of renewal pending funding

Job Purpose and Overview

Employers and individuals living with concussions and brain injuries often face barriers and challenges in the return to work process. The Return to Work program helps employers and employees navigate these challenges by offering tailored education programs as well as group and individualized support from an interdisciplinary team.

The Registered Social Worker will be a key member of the interdisciplinary team, working alongside an Occupational Therapist, an Employment Counsellor and an Outreach and Program Coordinator. This role focuses on supporting individuals' mental health and well-being as they return to work following a brain injury, including concussion.

Primary Duties and Responsibilities

1. Program Development and Implementation

  • Collaborate with the project team to research, develop, and implement an accessible, evidence-informed support program exploring topics including financial literacy, vocational skills, health literacy, mental health and coping strategies, relationships and communication for individuals living with concussion/brain injury.
  • Hosts educational webinars about the program to increase awareness about the program and recruit participants living with concussions/brain injuries.
  • Identify mental health, social, and systemic challenges affecting recovery and community reintegration.
  • Develop a tailored trauma-informed mental health support strategy.

2. Individual Mental Health Support

  • Conduct virtual psychosocial assessments and offer mental health support throughout the return to work process.
  • Provide case coordination and connect participants with community resources and services where appropriate.
  • Collaborate with the interdisciplinary team (occupational therapist and employment counsellor) to coordinate support plans.
  • Identify when participants require additional support or specialized intervention and provide referrals to OBIA Support Services or external providers as appropriate.
  • Offer ongoing guidance, motivation, and problem-solving support throughout the employment journey.

3. Program Outreach and Awareness

  • Hosts educational webinars to increase awareness about the program and recruit participants who are rejoining the workforce after a brain injury.
  • Support conference abstracts with the opportunity to present at conferences provincial wide
  • Support the development and implementation of a comprehensive outreach strategy in collaboration with the Project Team to promote the program across Ontario.

4. Documentation and Evaluation

  • Maintain accurate and up-to-date client records, including progress notes, employment plans, and outcomes.
  • Capable of analyzing data and research to support evidence-informed practice and program improvement.
  • Track program metrics and contribute to evaluation and reporting requirements.
  • Provide feedback and recommendations for program improvement based on client experiences.

5. Other

  • Assume such other duties as assigned by the CEO and/or designate.

Qualifications

  • Masters of Social Work (Required).
  • Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Minimum 5 years’ experience
  • Knowledge of concussion/brain injury, and its effects on individuals, families and professionals
  • Strong knowledge of system navigation and interdisciplinary collaboration.
  • Demonstrated competence in maintaining confidentiality and handling sensitive information.
  • Skilled in virtual service delivery and client-centered planning.
  • Knowledge of community resources, referral networks, and social service systems.
  • Ability to work both independently and collaboratively as part of an interdisciplinary team.
  • Deep commitment to equity, anti-racism, accessibility, and cultural humility.

Other:

  • Work arrangements (in-person, hybrid, or remote) will be determined by the employer based on operational needs and the employee’s location. Applicants who reside within approximately one hour of the office are expected to work primarily in person.
  • Travel within Ontario may occasionally be required for outreach or collaboration
  • Flexible work hours may be needed to accommodate participant schedules

This position is conditional upon confirmed funding and the successful execution of a funding agreement. Funding approval has been received, and final agreement terms are currently being finalized.
