About Us:

Reconnect Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. We pride ourselves on being at the forefront of client-centred care in our sector.

This role reports to the Manager, Compliance and Community Partnerships. As part of partnership between Reconnect Health Services and the North-West Ontario Health Team (NWT OHT), the Project Advisor will play a central role in supporting the Care Closer to Home (CC2H) outreach model in buildings and neighbourhoods in the North-West of Toronto. The Project Advisor will build trusted relationships with residents and support the development of socially connected, resident-led communities, with a focus on primary care attachment and connection to health and social services for seniors, newcomers and vulnerable residents.

Key Responsibilities

The Project Advisor will:

  • Identify residents without a primary care provider and promote primary care access and attachment
  • Build trust and awareness of primary care services
  • Identify residents’ health, social, settlement, and wellness needs
  • Connect residents by way of a referral to Reconnect Case Management services to:
  • Primary care access and attachment
  • Mental health and addictions supports
  • Social services, income supports, food security, and settlement services
  • Plan, organize, and coordinate CC2H site activities in collaboration with Reconnect and NWT OHT
  • Train, support and manage building resident Community Ambassadors
  • Contribute to the development of training and health promotion materials (including related to primary care) Build trust, empathy, and mutual support among residents
  • Support co-design of OHT and primary care initiatives and strengthen community capacity
  • Assist residents in navigating the health system, including referrals to needed supports
  • Facilitate social, recreational, wellness, and health-related programming for purposes of primary care outreach initiatives
  • Maintain documentation, reporting, and confidentiality per Reconnect and OHT requirements
  • Participate in program evaluation and approved research activities
  • Ensure compliance with Occupational Health & Safety legislation and policies
  • Additional duties may be required upon mutual agreement between Reconnect and the NWT OHT.

Minimum Qualifications

  • Relevant degree/diploma or equivalent lived and professional experience
  • Minimum 5 years of experience working with seniors, newcomers, or vulnerable populations in community settings
  • Strong leadership, problem-solving, and conflict resolution skills
  • Demonstrated cultural humility and equity-focused practice
  • Excellent communication, organizational, and time-management skills
  • Comfort working in evolving, community-based system environments
  • Fluency in English; additional languages an asset
  • Digital literacy (MS Office, Google Suite, virtual platforms)
  • Valid driver’s license, insurance, and reliable vehicle
  • First Aid and CPR-C (or ability to obtain within 60 days)
  • Preference for those with ties to local communities and neighbourhoods in North-West Toronto.

Working Conditions:

  • Monday to Friday 9:00am to 5:00pm
  • May be required to work evenings and weekends, depending on program needs.
  • Travel between key residential building sites and other sites within the OHT catchment area, is required.
