Program Coordinator, HART Hub

Full-time Contract position until September 15, 2026, 35 hours per week

Salary Range: $68,328 to $82,314, plus Benefits and HOOPP pension

Position Summary:

The HART Hub Program Coordinator provides day-to-day operational leadership and frontline supervision for the Downtown East HART Hub, a provincially funded, three-year demonstration project serving clients with complex needs such as homelessness, substance use, mental health, and social service challenges.

Operating within a Hub and Spoke model co-led by Fred Victor and the Downtown East Toronto Ontario Health Team (DET OHT), the Program Coordinator directly oversees a team of Community Health Workers and Outreach Workers delivering low-barrier, trauma-informed, mental health and addiction recovery services alongside intensive case management.

Responsibilities Include:

Staff Supervision and Leadership

  • Provide day-to-day operational supervision and leadership to the HART Hub team, including Community Health Workers and Outreach Workers.
  • Conduct regular one-on-one supervision meetings, performance evaluations, and support staff development.
  • Manage staff scheduling, workload distribution, and ensure adequate coverage.
  • Address day-to-day operational challenges and escalate complex issues to the Manager/Director as needed.

Program Operations and Service Delivery

  • Oversee the delivery of low-barrier, trauma-informed, outreach, mental health, and addiction recovery services.
  • Participate actively in the delivery and coordination of outreach, case management, and recovery services across Hub and Spoke locations.
  • Ensure services align with the HART Hub’s goals, funder requirements, and Regent Park CHC policies.
  • Facilitate smooth integration of services across the Hub and Spoke Partner sites while respecting role and program boundaries.
  • Troubleshoot operational issues and adjust workflows to meet changing client and community needs.

Partnership Coordination and Communication

  • Maintain regular operational communication with HART Hub Spoke Partners to coordinate outreach, case management, and service delivery.
  • Serve as an operational liaison with HART Hub Spoke Partners and other community agencies to facilitate day-to-day collaboration and service integration.
  • Build and maintain strong working relationships with program partners to coordinate resources and support seamless service delivery.

Data Management and Reporting

  • Monitor program indicators, identify trends, and recommend operational improvements.
  • Prepare and maintain accurate program records, documentation, and process financial documents as needed.

Qualifications and Skills required:

  • Bachelor’s degree in Social Work, Social Sciences, Community Development, Public Health, or a related field; or equivalent combination of education and experience
  • Minimum 3 years of experience coordinating programs or supervising frontline staff in community health, harm reduction, mental health, or social services
  • Experience working with populations experiencing homelessness, substance use, mental health challenges, or other complex social needs
  • Strong knowledge of harm reduction principles, trauma-informed care approaches, anti-oppression, and anti-racism frameworks.
  • Proficiency in data collection, program monitoring, and client record management systems (e.g., PS)
  • Flexibility to work occasional evenings or weekends as required by program needs
  • Demonstrates interpersonal, decision making, critical thinking and organizational skills
  • Commitment to valuing diversity, equity, partnerships and advocacy in keeping with Regent Park Community Health Centre’s Strategic Direction.
  • Knowledge of the Regent Park area and issues affecting families living within the Regent and Moss Park communities
  • Must be fully vaccinated for COVID-19 for the protection of clients and staff
