Physiotherapist Full-Time Regular Position (35 hours/week), Bargaining Unit - UFCW Local 175 Parkdale Queen West Community Health Centre (PQWCHC) is a dynamic, community-based health care organization that has historically served mid-west downtown Toronto, and now provides outreach services to across the entire city. Offering a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming, we are also committed to serving people who face barriers to health care, and we strive to reduce these barriers. PQWCHC is a multi-sited organization. General Responsibilities: We are seeking an experienced Physiotherapist who will deliver programs and services to clients with musculoskeletal conditions as well as chronic illnesses, including COPD and physical conditions related to mental illness. Experience with pelvic rehab and programs for people who are obese would be an asset. The physiotherapist will work within an inter-professional team to provide comprehensive assessment and treatment according to the standards of practice of the College of Physiotherapists of Ontario. Reporting Manager: Manager, Primary Care Specific Responsibilities: ï· Develop care plans by assessing, developing, implementing and evaluating treatments for clients with acute
and chronic musculoskeletal or neurological dysfunction, according to the College of Physiotherapists of Ontario guidelines and best practices
ï· Ensure continuum of care is maintained by educating clients and family members, if appropriate, with regard to the management of health care problems and re-evaluating physiotherapy programs considering the context of the clients’ home environment
ï· Provide consultation and capacity building to other centre staff and community agencies working with clients who may need physiotherapy care
ï· Develop, implement, monitor and evaluate, education and health promotion programs for individuals, families and the community, in order to foster self-help, mutual aid, and capacity building in the area of prevention or management of musculoskeletal and neurological problems according to community needs
ï· Assess for assistive devices (mobility equipment) and complete applications, where appropriate, for the Assistive Devices Program
ï· Develop and maintain formal and informal networks within the community to optimize health care service provision and address needs of community
ï· Maintain complete and accurate client records, assessment notes and required correspondence ï· Participate in chart reviews and case conferences, and make appropriate internal and external referrals ï· Participate in the Physiotherapy Network, including monthly meetings, resource development and quality
improvement initiatives. Qualifications:
ï· Master of Science in Physiotherapy from a recognized university and/or an equivalent combination of relevant education and experience
ï· Registered under the College of Physiotherapists of Ontario ï· Eligibility for membership with the Canadian Physiotherapy Association ï· Broad range of clinical experience (including chronic disease management) with different populations using a
variety of modalities
ï· Training in manual therapy, acupuncture and/or dry needling considered an asset ï· Demonstrated ability to work effectively in an inter-professional team environment ï· Experience in the use of computers and various software applications ï· Experience in program development, implementation, monitoring and evaluation ï· ADP authorization (or willingness to obtain) is considered an asset ï· Knowledge and awareness of issues related to homelessness, immigration, mental health, poverty and
discrimination
ï· Experience in a community-based setting working with marginalized groups ï· Knowledge of the health care/social service systems and resources available to clients ï· Strong communication, problem solving and conflict resolution skills ï· Ability to manage complex workload and remain flexible ï· Fluency in languages other than English is an asset ï· Experience providing care to Aboriginal clients is an asset ï· Experience in Aboriginal approaches and healing practices is an asset
