Position Summary
Reporting to the Supervisor, Assisted Living and Home Support Programs, the Personal Support Worker, assists tenants residing at 85, 110, and 115 The Esplanade. The primary responsibility of the PSW is to provide essential support to tenants who are frail or ill, enabling them to successfully complete their activities of daily living (ADLs) and support Aging at Home.
The PSW also provides important companionship and emotional support. The PSW is responsible for observing and reporting any changes in residents’ health or social status and for keeping records of each resident’s service plan. The PSW should be capable of working in a cluster care model, supporting vulnerable individuals of diverse social, cultural and language backgrounds.
Duties and Responsibilities
Personal Support services:
- Follow the care plan, observe and report any substantial findings and/or changes in the client’s behaviour and condition to the Supervisor or other appropriate member of the team.
- Assist with or complete personal care tasks such as bathing, foot care, dressing/undressing, toileting, skin care, mouth care, hair care, etc.
- Assist residents with ambulation needs including transfers, turning/positioning, and exercise.
- Assist residents with taking pre-measured medications.
- Essential Homemaking
- Completing household management tasks such as laundry, food access, bed-making, washroom cleaning, mopping personal item organizing and assisting with appointments.
Communication:
Follow the care plan, observe and report any substantial findings and/or changes in the client’s behaviour and condition to the Client Support Coordinator, Supervisor, or other appropriate member of the team.
Record-keeping
- Document services provided at the end of each shift using the system provided (i.e. mobile app, paper, and so on).
Support Systems and Professional Development:
- Attending staff meetings, performance reviews, and supervision meetings as scheduled.
- Participating in briefings to relief staff during shift changes.
- Attending in-house training opportunities as scheduled.
- Other duties as required.
Qualifications
- Personal Support Worker certificate or equivalent training and experience.
- Nursing background an asset.
- Minimum of 1 year experience as a PSW (required).
- Current First Aid & CPR Certification an asset.
- Experience providing general care and support.
- Knowledge of home care services.
- Excellent interpersonal, personal care, and household management skills.
- Strong verbal and written English language communication skills; fluency in additional languages, relevant to the communities of service, will be considered an asset.
- Knowledge of and skill in documentation of care provided.
- Knowledge of Gerontology and Mental Health issues an asset.
- Ability to respond and act quickly in emergency situations.
- Demonstrated ability to work both independently and collaboratively as part of a team.
- Previous experience working with seniors or people with health and mobility challenges is a must.
- Available to work all shifts (days, afternoons/evenings, and nights).
- Must be qualified to work in Canada legally for the employer.
- Able to provide a Vulnerable Sector Screening check with a ‘Clear’ or satisfactory result that meets the operational requirements of the Agency.
- Must have up-to-date vaccinations against COVID-19 with a vaccine that has been approved for use in Canada.
