Cota is an accredited not-for-profit organization that has been providing community-based support to people living with mental health and cognitive challenges for over 50 years. At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. These services include, but are not limited to, case management, supportive housing and adult day program services. Role Overview
The Manager, Community Health Services is accountable for providing leadership in the development and delivery of high-quality service user and family-centred services that meet service recipients' needs, ensures their safety, and works within a quality improvement framework to identify opportunities for enhancing quality, and quantitative outcomes of the services.
This position is initially responsible for a portfolio that includes, but may not be limited to, managing our: Toronto West Integrated Service Team (TWIST) and Boarding Home West Program.
Boarding Home West Program - The Boarding Home Program provides supports to tenants living with a serious mental illness residing in boarding homes supported by Habitat Services and/or apartments operated by Habitat Housing. The team consists of nine (9) Site Support Workers and one (2) Mental Health Case Manager. The Site support workers provide social recreational and educational groups/support to tenants and staff within the homes, while the Case Manager provides more individualized supports for those tenants requiring it.
Toronto West Integrated Service Team - TWIST provides individualized support to service users living with a serious mental illness in West Toronto. The multi-disciplinary team consists of eight (8) Mental Health Case Managers, two (2) Addiction Specialist Case Managers, two (2) Acquired Brain Injury Case Managers, one (1) Psychogeriatric Case Manager, two (2) Dual-Diagnosis Case Managers, and one (1) Community Supportive Housing Worker. The team works collaboratively with their service users to provide goal-focused, person-centered supports to assist them to live active and fulfilling lives within the community.
Responsibilities:
The Manager, Community Health Services will be responsible for the following:
- The Manager is accountable for providing leadership direction, clinical and operational oversight and guidance to the program(s) and in ensure our services are rooted in Recovery Oriented, Housing First, Harm Reduction and Anti-Oppression principles
- Collaborating with Directors, and/or other members of the leadership team in relation to leading Program Development/Quality Improvement activities within the organization
- Oversee the recruitment, training, supervision of staff and provide administrative and clinical support to frontline service providers
- Establish new and ongoing positive relationships with community partners and external stakeholders
- Represent Cota at all external stakeholders, committees, groups and/or tables
- Lead and develop a program evaluation
- Collect and analyze data based on the goals of the program
- Identify and implement quality improvement initiatives as required
- Provide oversight of the management of program budgets with internal support
- Support a high-functioning team, manage staff wellness and performance
- Schedule staff and conduct regular performance evaluations of team members
- Play a lead role in developing standards/guidelines and providing education of program specific operating standards/guideline manuals
- Keep the Director abreast of information and issues related to the delivery of services
- Provide effective problem solving, identifying and analyzing issues and select the most suitable solutions
- Authorize/process staff expenses, mileage claims and vacation requests
- Approve program related expenditures (per applicable policies and guidelines)
- Authorize/process staff attendance for payroll
- Participate as a member of the Clinical Leadership Team in planning / developing of the team's goals and objectives, as well as participating in a number of organization-wide activities
- Act as a member of the Cota Leadership Forum
Qualifications:
Education and Experience:
- Degree/Diploma from a recognized University or College in a related Health Care or Social Service discipline
- Strong clinical skills with at least 5 years' experience providing community-based services to individuals with serious mental illness, addictions, and complex health conditions
- Three years of supervisory and program management experience with an interdisciplinary team will be required
- Experience in hiring, training, supervising and retaining qualified staff
- Experience in networking and working in partnership with other groups/agencies
- Experience working in a unionized environment in a supervisory capacity
- Current registration in good standing with the respective professional college (e.g., OT, SW, RN, etc.) will be considered an asset
Knowledge & Skills:
- Demonstrated excellence in organizational, interpersonal and communication skills; team building and coaching abilities are essential
- Knowledge of Intensive Case Management models is required
- Strong knowledge of evidence-based practices used in the mental health and addiction field, including recovery practices and harm reduction
- Solid team player with a demonstrated ability to work with flexibility and efficiency in a fast-paced environment
- Ability to work effectively with individuals with complex needs and multi-system involvement
- Ability to maintain a positive, solution-focused approach when responding to conflicts or problems
- Proven cultural competence and ability to work effectively with diverse populations
- Knowledge of information management, quality improvement, benchmarks, and evaluation activities
- Strong writing skills and attention to detail in preparing and/or reviewing reports and financial data
- Familiarity with TREAT electronic health record, OCAN and OPOC reporting will be considered an asset
- Proficient in Microsoft Office - Word, PowerPoint, Excel, Outlook, SharePoint
Other:
- Flexibility and willingness to work varied 'regular' work pattern to suit the needs of the program. Evening and early mornings shifts may be required, and the ability to work one - two weekend days a month
- Member of the organization's Manager-On-Call system on a rotating basis
Benefits:
Upon successful completion of a three-month period of employment, permanent full-time and permanent part-time staff working more than 20 hours per week as well as temporary full-time (with a contract for at least 1 year) and temporary part-time staff (with a contract for at least 1 year and working more than 20 hours per week) will be eligible for employer-paid group insurance benefits. Permanent full-time and permanent part-time staff working more than 20 hours per week are also required to enroll in employee-paid long-term disability coverage.
