
Lead, Integrated Health Services
Our client is a not-for-profit organization expanding and strengthening its community-based health services. They are seeking a Lead, Integrated Health Services to provide leadership in the planning, delivery, and evaluation of services that support individuals and communities with complex and evolving needs.
Position Details
This is a permanent, full-time position, remote within Canada.
This role reports to senior leadership and works collaboratively across teams and community partners.
As our team works across multiple provinces and territories, some flexibility will be required to accommodate different time zones, particularly for bi-weekly team meetings. Outside of these times, you may work within your local time zone.
Compensation and Benefits
Salary range: $118,147 to $155,461, based on experience and qualifications.
Salaries are established within the posted range based on consideration of the candidate’s qualifications, experience, and internal equity.

Key Responsibilities
Maintains a strong awareness of current issues and trends in health and community-based care, as well as current and emerging evidence and population needs. Proactively identifies improvement opportunities based on this awareness.
In collaboration with senior leadership, supports the development of an integrated and networked service delivery model that improves outcomes for individuals and communities in an effective and sustainable manner. Works with internal and external partners to:
- Improve outcomes for individuals with complex health and social needs
- Support access and navigation of community-based services for individuals, families, and caregivers
- Engage individuals, families, and communities as partners in care
- Promote health and prevent illness across targeted populations
Supports the development and implementation of practice standards, policies, and guidelines related to community-based care.
Plans, evaluates, monitors, and reports on programs and services to ensure effective delivery within the assigned portfolio of responsibility.
Works collaboratively across teams to support integrated service delivery and alignment with organizational standards.
Leads and supports a team, including recruitment, onboarding, performance support, and ongoing development.
Develops and monitors operating budgets, reviews expenditures, and supports fiscal accountability.
Manages implementation of approved plans, identifies risks, and monitors progress toward deliverables.
Participates in quality improvement initiatives and supports system improvements across services.
Builds strong relationships with community partners, including Indigenous and other external partners.
Integrates knowledge from individuals, families, and people with lived experience into service planning and development.
Supports contract management and procurement processes as required.
Maintains a healthy and safe work environment.
Performs other related duties as assigned.
Qualifications
Required
- Bachelor’s Degree in a health-related discipline, education, or other relevant field
- Five (5) to six (6) years of recent, related experience, including leadership experience, or an equivalent combination of experience and demonstrated leadership
Preferred
- Master’s Degree in a related field is considered an asset
- Training or experience in mental health, substance use, or community-based care
- Experience working with diverse populations and complex care needs
- Experience in program development, evaluation, or quality improvement
- Familiarity with and training in trauma-informed and culturally safe approaches to care
