Journey Facilitator

Position Type: Full Time – 37.5 hours per week 6-Month Term

Location: Vancouver Lower Mainland

Compensation: $29.17 hourly with excellent benefits after probation (2024 rate)

We are searching for a Journey Facilitator (6-month term) to make a difference in the lives of adults with developmental disabilities. Gain the gratification of knowing you are instrumental in helping individuals create their success!

About YOU:

  • You hold a strong commitment to protecting the Human Rights of our persons served. This commitment comes from your grounded philosophy of supporting people with challenges living and working in the community.
  • You hold a diploma in Social work, Social Sciences, Human Services or an equivalent combination of education and experience
  • You have minimum 2 years of experience working in highly collaborative environment and with multi-stakeholder groups.
  • You have experience working with people with diverse and unique communication styles
  • You aren't a tech guru, but you can help your grandma understand how to send emails. Learning new technology and data platforms doesn’t daunt you.
  • You know your way around the local community, having knowledge of the resources and services available.
  • You are a confident communicator, with excellent interpersonal and communication skills
  • You are organized, mature, and a team player
  • You MUST have the following:
    • Current Emergency First Aid with CRP level A or C
    • Mandt System training,
    • Negative TB test result
    • A clear Criminal Record Check for working with Children and Vulnerable Adults

YOUR Typical Day:

  • You provide information on all services offered by the Association to persons served and other stakeholders
  • You facilitate a service journey experience from a person centered approach, with an emphasis on self-determination and individuality, participate in intake meetings
  • You conduct discovery and support persons served in co-designing their service journey using appropriate planning tools
  • You make decisions, recommendations and coordinate service journey based on desired individual outcomes.
  • You design and implement the strategies, principles and frameworks for Explore within the Association
  • You collaborate with persons served, their networks, and other Association services to address any issues that arise during their service journey with support from the Journey Coordinator
  • You input data in a timely manner and ensure that it is accurate and current including person served information on ShareVision
  • You monitor persons served journey experience by maintaining regular communication with person served, their networks and services.
  • You participates in meetings with other stakeholders as required
  • You identify policy, procedural or structural changes within the service delivery system in order to support individuals
  • You participate in the development and presentation of presentation to stakeholders, in the development, implementation and analysis of service evaluation and impact
  • Completes all required documentation and reporting
  • You educate staff within the Association and other stakeholders to understand the frameworks and principles of the Explore model of service in order to facilitate a cohesive network of support
  • You may represent the Association at a professional and community level (e.g. committees) and participates in the organization’s special events and marketing
  • You seek and mobilize resources/partnerships/community assets to support the needs of individuals throughout their Explore journey
  • You monitor the number of people they are case managing and the intensity of support, informing the Journey Coordinator when there is capacity for additional referrals
  • You are able to work flexible hours including some evenings / weekends, as determined by person served needs and schedules
