With a holistic circle of care approach, Surrey Place brings together caring clinicians, staff, valued partners and other experts to collaborate for the client’s best interests. The organization works with clients throughout their lives, creating inventive and innovative programs so they can keep building their skills, grow their self- confidence and make their sight even higher. Since 1962, Surrey Place has supported people with varied and complex needs in the Toronto region and more recently, Northwestern Ontario. Surrey Place is accredited with Exemplary Standing by Accreditation Canada through the healthcare program. Position Description The Surrey Place Family Advisory Council Volunteer (FAC) brings together caregivers and family members of children and adults living with intellectual or developmental disabilities and/or autism to share their lived experiences and help shape the future of services at Surrey Place. As a FAC Council Volunteer member, you’ll have an opportunity to share your voice, connect with other families, and help make a meaningful difference. Together with Surrey Place staff and fellow caregivers, you’ll help identify priorities, highlight gaps in services, share ideas and provide recommendations that can contribute to positive changes for individuals and families. We are looking for passionate caregivers and family members who want to use their experiences to help improve the Surrey Place experience for others. Your perspective matters, and your voice can help create meaningful change. Our Goals Include:

 Prioritizing concerns and matters that are important to Surrey Place families and caregivers  Sharing family and caregiver perspectives to help inform clinical practices and service delivery  Identifying gaps in services and suggesting concrete, practical solutions  Guiding Surrey Place staff in service enhancement and improvement  Building community among Surrey Place caregivers and family members  Supporting meaningful and positive changes for individuals and families accessing Surrey Place

Qualifications We are looking for caregivers and family members who:

 Are a caregiver or family member of a child or adult living with an intellectual or developmental disability and/or autism

 Are interested in sharing their lived experience and perspective  Are passionate about improving services and support for individuals and families  Are comfortable participating in group discussions and providing constructive feedback  Are willing to listen to and respect diverse perspectives and experiences  Can work collaboratively with other caregivers, family members and Surrey Place staff  Can commit to participating in scheduled Family Advisory Council meetings  Are committed to maintaining confidentiality and respecting the privacy of others

Previous committee or volunteer experience is not required. The Family Advisory Council values the lived
