Position Summary

In collaboration with the Director of Community Partnerships and Programs, the Coordinator will identify opportunities and build partnerships to enhance supportive cancer care services in the community, promoting equitable access to supports and resources. The Coordinator will collaborate with a multidisciplinary team to improve client care and improve health outcomes related to the social determinants of health. They will report directly to the Director of Community Partnerships and Programs.

Responsibilities

  1. Community Partnerships
    • Plan and implement strategies to achieve better access to cancer related health priorities and community health supports in collaboration with community partners.
    • Assist Director of Community Partnerships and Programs with the coordination of a local Community of Practice for health and social care providers in community supportive cancer care.
    • Assist Director of Community Partnerships and Programs with the administration of a Community Advisory Group.
    • Facilitate and steward collaborative relationships with underserved communities and the organizations that work alongside them.
  2. Health Equity Work:
    • Identify and address barriers to healthcare access for underserved and marginalized communities.
    • Develop and implement health equity initiatives aimed at reducing disparities in accessing community cancer care.
  3. Collaboration and Advocacy:
    • Act as a resource for community groups by attending, presenting at and/or facilitating workshops, webinars, information sessions or focus groups.
    • Participate in community outreach and education programs to raise awareness about cancer resources and support services.
    • Develop health and community-based projects in conjunction with other team members that are responsive to identified needs, and in accordance with the strategic direction of the organization.
  4. Reporting:
    • Prepare regular reports on activities, outcomes, and challenges to the Director of Community Partnerships and Programs
    • Ensure compliance with all relevant regulations and standards of practice.
  5. Other
    • Adhere to The Ottawa Cancer Foundation’s organizational values and policies, and procedures.
    • Attend internal meetings, events and staff trainings as appropriate.
    • Contribute to and participate in TOCF’s philanthropy efforts, including attending events as required.

Qualifications

  • Education: Post-secondary degree in Social Work, or equivalent education and experience in a related profession such as community development, health promotion or program development.
    • Experience working in oncology or chronic illness an asset.
    • Thorough understanding of community engagement and development principles.
    • Experience working with low-income or/and underserved community members.
    • Experience working with diverse community groups and teams.
    • Familiarity and experience with using a trauma-informed approach an asset.
  • Language: Written and spoken English language proficiency required. Bilingual or multilingual abilities are an asset.
  • Skills and Experience:
    • Strong understanding of health equity principles and practices.
    • Excellent interpersonal and communication skills.
    • Ability to work independently and as part of a multidisciplinary team.
    • Familiarity working with community-based health and social care programs.
    • Proficient in Microsoft Office Suite (Excel, Word, Outlook).

Working Conditions

Hours: Regular office hours (8:30am – 4:30pm) with some evening or weekend hours required to meet client needs or to support donor events or outreach activities.

Location: [On-site/Hybrid] On-site 4 days a week, one day remote. This position may be required to work 1 day per week on-site at a partner location in the west end of Ottawa, in addition to 3 days on-site at 1500 Alta Vista.

Travel: Travel to partner agencies, hospitals, or community settings will be required.
