Chiropodist – HART Hub Part-Time Regular Position (0.6 FTE: 21 hours/week), Bargaining Unit - UFCW Local 175
Parkdale Queen West Community Health Centre (PQWCHC) is a dynamic, community-based health care
organization that has historically served mid-west downtown Toronto, and now provides outreach services to
across the entire city. Offering a broad range of services, including primary health care, dental care, harm

The HART Hub Chiropodist is part of the HART HUB services which will work closely with the mental health, substance use care (continuum of treatment, recovery, harm reduction), and health promotion teams to ensure cross-team and cross-site collaboration. The Chiropodist will work with the team to contribute to the growth and development of the HART Hub program which will include the development of processes and pathways for HART Hub clients. Reporting to the HART Hub Manager, the Chiropodist will participate in larger HART Hub partnerships led by the Centre, which may include, but not limited to CAMH, LOFT, and The Neighbourhood Group. The Chiropodist will support clients and share knowledge. General Responsibilities:
• Conducts assessment and treatment of foot conditions, including but not limited to diabetic foot care, nail
care, biomechanical assessments, custom orthotic casting and dispensing, wound care, measuring for
compression stockings, and conducting procedures such as PNA, TNA and soft tissue lesion excisions
• Works closely with staff to provide foot care within a broader multi-disciplinary team; works particularly
closely with the diabetes nurse and diabetes dietitian
• Builds trust and rapport with clients to promote effective service use
• Provides health education, harm reduction education, and advocacy to HART Hub clients
• May provide services at designated outreach locations in the community and in the mobile health unit
• Maintains client/patient files through PQWCHC’s EMR, encounter information, correspondence, clinical
protocols, and other data to meet the requirements of the funders, the clinic and regulatory bodies
• May act as supervisor/consultant to health care students and participates in the learning of students from
other disciplines
• Participates in the design, implementation and evaluation of research studies in consultation with the
Management team and other staff
Qualifications:
1. Graduate of a recognized educational institution for Podiatry/Chiropody; certificate of practice in good standing from the College of Chiropodists of Ontario to work to full scope, including injection and prescription privileges
2. Demonstrated ability to fit for orthotics 3. Experience in community-based practice and working with marginalized populations. 4. Ability to work as part of an interdisciplinary team 5. Excellent communication and problem-solving skills 6. Ability to work in a flexible manner to ensure client access to service 7. Good organizational and time management skills 8. Ability to speak a second language is an asset
Compensation: $69,599 to $81,343 per annum, plus Group Benefits and HOOPP (Health Care of Ontario
Pension Plan). All pro-rated to a 0.6 FTE.
