PURPOSE

Working in an interdisciplinary team using a client centered and self-management approach, Registered Dietitian provides nutritional assessment, monitoring, and counseling for clients with type 2 diabetes or pre-diabetes. The RD plans and in conjunction with community partners, implements and promotes community-based diabetes prevention and management programs. The RD works out of the Well Fort and various off-site locations within Brampton and Malton communities. While the proportion of time allocated to different functions may vary according to service needs, the role encompasses a combination of direct care, health promotion and education, advocacy and community development.

RESPONSIBILITIES

  • Co-facilitates group diabetes education sessions for adults with type 2 diabetes and those
  • who have pre-diabetes by disseminating appropriate nutrition education that is consistent with
  • clinical best practices for prevention and management of diabetes, ensuring a safe and
  • barrier-free environment for learning and sharing of experiences, and ensuring client needs are met in a relevant way.
  • Conducts nutrition assessments, including taking a complete medical and social history and works with clients to develop nutrition plans, taking into consideration such factors as psychosocial, economic, functional and family status
  • Provides individual nutritional counseling to clients with type 2 diabetes and pre-diabetes utilizing a self-management approach, thus working collaboratively with clients to work towards client identified goals.
  • Uses a wide range of modalities for teaching/counseling to enable clients to acquire the knowledge, skills, and confidence needed to effectively manage diabetes.
  • Ensures continuity of care by maintaining up-to date medical records, communication with the client’s primary care provider and other service providers, as required..
  • Provides health related equipment or supplies to clients and advises on proper use.
  • Identifies trends or issues related to the health status of priority groups living with diabetes and uses this information in planning and advocacy work.
  • Provide care delivery at any of the office sites as program needs dictates Other duties as assigned.

QUALIFICATIONS, EXPERIENCE & SKILLS

  • Undergraduate degree in food and nutrition from a recognized university or equivalent.
  • Completion of a Canadian Dietetic Association accredited Dietetic Internship
  • .Registered with the College of Dietitians of Ontario in good standing
  • A minimum two years experience working as a diabetes educator or working in a community-based health promotion setting working in a similar position.
  • Certified Diabetes Educator (CDE) is an asset or willingness to acquire.
  • Demonstrated experience in health promotion program planning, development, implementation, and evaluation.
  • Demonstrated experience in facilitating groups and providing individual counseling using a client centered approach.
  • Demonstrated commitment to best practices, ongoing learning, and client centered care.
  • Experience working with communities and individuals who face barriers accessing traditional health care institutions due to poverty, discrimination, mental health and substance use issues.
  • An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.
  • Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
  • Experience working with confidential and time sensitive documents and information.
  • Excellent interpersonal skills and the ability to multitask in a fast paced environment.
  • Experience working in a value based non-profit or social services organization.
  • Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
  • Demonstrated proficiency in use of computers and various computer software applications, such as Electronic Medical Record (EMR).
  • The ability to speak languages relevant to the residents of the organization’s community is an asset.
  • Holds a valid Ontario driver’s license, and access to a reliable vehicle with insurance for responsive care delivery at any care delivery location.
