Name Name under which applicant has been educated or employed. Maiden name.  
Address Present and previous addresses in Canada. No inquiry provided. Foreign address which may indicate national origin.  
Age   Information revealing specific age. Birth certificate may be requested but not used to discriminate.
Sex Inquiry permitted only in the case of a domestic employed and living in a single family home or with an exclusively religious or ethnic organization where sex is a reasonable occupational qualification. Any inquiry as to gender; different or coded application forms for males or females.  
Marital Status Only questions pertaining to the willingness of applicant to travel to other areas of the province or country. May be asked but not used to discriminate. applicant is pregnant, on birth control, or has future child-bearing plans. Applicant's marital status, spouse, number of children or dependents, child care arrangement
Birthplace Nationality Ethnic or
National Origin
No inquiries permitted. Any inquiry revealing birthplace, applicant's nationality or nationality of parents, grandparents, relatives, or spouse, or ethnic origin, mother tongue.  
Citizenship 'Are you legally entitled to work in Canada?' Any inquiry into citizenship status that would tend to divulge nationality, ancestry or place of origin. Request for documentary proof of legal entitlement to work in Canada.
Military Service Inquiry into Canadian military service only. Inquiry into all military service.  
Languages Spoken Languages the applicant speaks, reads or writes. Inquiry into mother tongue or where knowledge of language was obtained.

Color
No inquiry permitted. Any inquiry which would indicate race, color, or complexion, including color of eyes, hair or skin.  
Photographs Not permitted. Any request for a photograph. Acceptable after hiring if for purposes of identification.
Religion Creed If employment is with a strictly religious organization that is not operated for private profit, and is operated primarily to foster the welfare of a religious group, and with respect to persons of the same religion. Any religious holiday for which the employee will require leave of absence. Any inquiry revealing religious denomination, affiliation or preference, including willingness to work on any religious holiday, church holidays observed, religious dress.
Political Belief No inquiry permitted. Any question revealing political belief, affiliation or preferences.  
Education Academic, vocational, professional or technical school attended, and nature and level of education received. Specific request for information relating to attendance at religious denominational schools which could be used for discriminatory purposes.  
Organizations Membership in any organization with the qualification that applicant is not required to list any clubs or organizations that indicate race, religion, color, age, sex, marital status, ethnic or national origin, political belief, or physical or mental handicap. If individual wishes to join workorganized activities or teams, etc. Request that applicant list all clubs, organizations where membership is held.
Relatives Name and address of person to be notified in case of emergency. Name and address of next of kin to be notified in case of emergency.  
References Many question references, including previous employers, about information pertinent to job performance. Who referred applicant for a particular person. belief, or physical or mental disability. Any inquiry from a reference that would reveal applicant's race, religion, color, sex, age, marital status, ethnic or national origin, political

Defect or Disability Has the applicant a handicap or health problem that reasonably precludes performance of duties for the position under application? Any general inquiry into the nature of a physical or mental disability or health problems.
Work Schedule Willingness to work required work schedule. Any inquiry about willingness to work on any particular religious holiday. Any religious holiday for which the employee will require leave of absence.

Inquiry permitted only where directly required by the job or jobs sought. Any inquiry which may be discriminatory if it screens out disproportionate numbers of minority group individuals or women and cannot be shown to be essential to the performance of the job.  
Miscellaneous   Request applicant to consent to a personal investigation.
