Name Name used in previous employment. Last name, first name or initial, middle name or initial. 'How would you like to be addressed, Mr., Mrs., Miss, Ms., or as above?' Full name, changed by court order or other Full name. Inquiry into maiden name and/or previous name, whether it has been wise. Indicate whether 'Mr., Mrs. or Miss'.
Address Past and present addresses in Canada and length of residence. Inquiry into foreign address.  
Sex   Any inquiry. Any inquiry.
Marital Status   Any inquiry. Any inquiry as to applicant's spouse and/or number of children. Information required for tax or insurance purposes.
Age 'Are you over 16 and under 65 years of age? If not, please state age.' Any inquiry, e.g., request for birth certificate or baptismal record, that may reveal age of applicant. Verification of age.
National or
Ethnic Origin
Race and/or Color
  Any inquiry that may reveal applicant's racial origin, e.g., color of hair, eyes,,  
Photographs   Request for photograph or taking of photograph. Request etc. for photograph if required for identification purposes.
Education Highest grade attained. Post-secondary school education, e.g., B.A., etc. Names and locations of schools, as such may reveal national and/or ethnic origin of applicant. Any inquiry into the racial and/or religious affiliation of school(s).  
Languages Inquiry into proficiency of official languages. Other language skills, only if job related, e.g., Spanish instructor. Mother tongue. How and where language skills were attained.  
Religion

Creed
Willingness to work required work schedule. Religious affiliation, or customs. Availability to work on specific religious holidays. Request references and/or recommendation from religious leaders, e.g., priest, rabbi, etc. Inquiry about religion to determine when leave of absence might be required for observance of religious holidays.
Military
Service
Inquiry into Canadian military service. Inquiry into all military service.  
Height and weight If height/weight is essential for job performance. Any inquiry (unless it is job related).  
Physical
Disabilities
'Do you have any physical disabilities that you wish us to take into consideration for placement?' Inquiry into all physical handicaps and/or health problems. The employer may indicate on application form that job offer is conditional on applicant passing a medical exam.
Relatives   Names and addresses of closest relatives. Name and address of next of kin to be notified in case of emergency.  
Organizations If option given, do not mention organizations which may reveal applicant's race, sex, marital status, etc. List all organizations where membership is held by applicant. Name and address of person to be notified in case of emergency.
