After Hiring
Name Name under which educated or employed. Maiden name.  
Address Address in Canada. Inquiry into foreign address which would indicate national origin.  
Age Are you between the ages of 19 and 65 years? Any request for specific age. Birth certificate may be requested for personnel reasons.
Sex If necessary because of a bona-fide qualification or in the case of a domestic employed and living in a single-family home. Any inquiry as to sex.  
Marital Status No inquiry permitted. Any inquiry as to marital status, spouse, number of children, or pregnancy. Acceptable for personnel purpose.
Photograph Not permitted. Any request
for photograph.
Acceptable

Languages Inquiry as to whether applicant is fluent in a language required by the job. Mother tongue.  
Religion
or
Creed
No inquiry permitted. Any inquiry to identify religious beliefs or customs. Request for reference from clergy. Religious holidays for which the employee will require leave.
Race
Color
No inquiry permitted. Any inquiry which would indicate race or color including color of eyes or hair.  
Physical Handicap Inquiry as to whether applicant has a handicap or health problem affecting ability to perform the job applied for. A job offer can be made contingent upon applicant passing a job-related medical. General inquiry into physical handicaps or health problems.  
Nationality 'Are you legally entitled to work in Canada?' Any inquiry as to birthplace. Proof of eligibility to work in Canada.
Political
Belief
No inquiry permitted. Any inquiry as to political belief.  
Mental
Disability
If the applicant has a mental disability which might hinder ability to perform in job applied for, a job offer can be made contingent upon passing a job-related medical. Any inquiry into all mental disabilities.
