Name Name used if previously employed or educated under a different name Inquiry about maiden name. Any inquiry into previous name where it has been changed by court order or otherwise. These recruitment and interviewing guidelines apply only to inquiries prior to hiring.
Address Past and present addresses in Canada, length of residence. Inquiry into foreign addresses which would indicate national origin.  
National Origin,
Place of Origin,
Birthplace,
Ancestry
'Are you legally entitled to work in Canada?' Any inquiry into place of birth, national origin, place of origin, or ancestry, e.g., birth certificate or baptismal certificate, mother tongue. Any inquiry into national origin, place of origin, place of birth or ancestry of parents, grandparents, relatives or spouse. Is applicant native born or naturalized, date citizenship received, proof of citizenship? Request for birth certificate
Age Is applicant of legal working age? Questions about age, or date of birth. Any request for a birth or baptismal certificate.  
Sex   Any inquiry into the sex of the applicant. Use of different application forms and questions for males and females.  
Race
Color
  Any inquiry which would indicate the race or color of the candidate, including color of hair, eyes or skin.  
Religion Applicant's willingness to work a specified schedule. Any questions which would reveal religious denomination or customs, i.e., church attended, religious holidays, or religious dress. Request a reference or recommendation from a pastor, priest, minister, rabbi or other religious leader.  
Marital   Marital status of the applicant, i.e., single, married, divorced, separated, widowed, remarried. Questions about the applicant's spouse.  
Mental
Disability
  Any inquiry regarding mental disability, as defined in the Code.  
Physical Disability   Any inquiry into physical disabilities (see definition of 'physical disability' in the Code), health problems, injuries. Any inquiry into height and weight. A request for a medical exam or a medical report. Any inquiry into workers' compensation claims.  
Education Academic, professional or vocational schools attended. Language skills, such as written and verbal skills acquired in foreign languages. National, racial or religious affiliation of a school attended. Applicant's mother tongue or how foreign language ability was acquired.  
Relatives Names and addresses of person to be notified in case of emergency. Names and addresses of relatives or next of kin.  
Organizations List clubs and organizations to which applicant belongs, except those that indicate race, color, religion, national origin, ancestry place of origin, physical disability, sex, age or marital status. Request that applicant list all clubs and organizations where membership is held. Specific inquiry into clubs and organizations which would indicate applicant's race, color, religion. national origin, ancestry, place of origin, physical disability, sex, age or marital status.  
Photographs   Any request for applicant's photograph.
